I/We further agree to indemnify and hold harmless the City of Chicago, DuSable Museum, the Chicago Park District, event staff, volunteers, vendors and corporate sponsors associated with this event from any and all liability including, but not limited to: theft, personal injury, bodily injury, strike, public enemy or act of God and to indemnify it for any damages arising from my/our conduct at the event. I/We further agree to abide and comply with the rules, regulations and requirements as specified in the DuSable Museum Arts & Crafts Festival guidelines, City of Chicago and Chicago Park District and that this application does not guarantee a space. If required, I/We will provide a sample of my work. I/We further grant permission to the DuSable Museum to use stories, photographs or videos taken of myself and my artwork taken during the Festival for purposes of promoting the Festival this year and future years.
