EXPERIENCE:
My entire experience as a nurse is being focused on hemodialysis considering that right after I received
my license as a registered nurse in the Philippines. I already have an instinct to find as slot for me to be
enrolled at Asia Renal Care which is now known as Fresenius Medical Care for a Hemodialysis Training. I
took and passed their exam so I started the training. I completed it for two month,s then I took the
Accreditation Exam to be a Certified Renal Nurse. I passed the exam and then I applied in ZaNorte
Medical Center in Sicayab, Dipolog City. Luckily, I am directly assigned in the Hemodialysis Unit from
September 24, 2010 up to the present.
I could say that it is my passion to be a hemodialysis nurse and I want to best in the world there is. I
have encountered patient up to their tomb. We cater patients with different disease conditions including
Diabetes Mellitus, Hypertension, Rejected kidney transplant, Urates Nephropathy, Acute Kidney disease
due to kidney injury, Chronic Glumerolunephhritis, Multi organ failure, unstable cardiac conditions,
heparin free hemodialysis, also pre and post operative patients who undergo hemodialysis. I have wide
experience in the 4 walls of our clinic. We also assist insertion of Internal Jugular and Femoral Catheter.
My work shift is from 6am to 2 pm and 12noon to 8 pm from Monday to Friday and we also cater
Saturdays and Sundays for emergency cases without overtime pay. In a week or two I have straight 6 am
to 2pm duties from Monday or Friday otherwise 12 noon to 8 pm. During the shift, we take care of
patients with mixed conditions. Mostly, when there is a critical patient who is admitted we prioritize them
in the morning session. We dont categorize patient according to their conditions but they are given slots
available. Some patients have 3 times hemodialysis on Monday, Wednesday and Friday. Some have two
sessions per week Monday and Thursday, Tuesday and Friday also Tuesday and Thursday. We have 38 a
census of 38 patients. We have six
2 years and 11 months of solid exposure is educational, fun and challenging. My life as a nurse is devoted
to hemodialysis and I guess if given the chance to be exposed to King Fahad Armed Forces Forces
Hospital I would even be more challenged to face whatever there is. If not now, may be soon.

COMMENT

I. PERFORM THESE PROCEDURES:


Initiation of dialysis

Set up and Priming

As soon as the patient is


diagnosed by the Nephrologist to
have Acute or Chronic Kidney
Disease and with High Creatinine
and Blood Urea Nitrogen Levels.
The staff nurses are informed
that there is a possibility for an
incoming patient to be inserted
with Intra Jugular Catheter.
The significant other (family
member) is being called up from
the ward and is counseled by the
nurse on duty regarding
hemodialysis. First, assessment
is being done considering the
beneficence of the procedure to
the patient. Nurses go to the ICU
or any wards where the patient
is assigned to examine the
condition of the patient. The age
is being looked after, the
financial capacity of the family
to sustain hemodialysis or to
have a kidney transplant, then
the readiness of the whole
family.
Second,
patient
education
is
being
given
considering the advantages and
disadvantages of hemodialysis
including a touch of kidney
transplant. Third, the estimated
cost of hemodialysis is being
revealed as well as the possible
schedule available. Prior to
initiation there is a consent for
intrajugular catheter insertion
as soon as the family and the
patient is okay to proceed with
hemodialysis. Prior to insertion
the HBSAg and HCV results are
being taken to know if the
patient is infectious or noninfectious, laboratory results are
also checked especially cbc to
know if there is a need to
transfuse blood. When results
are available, the nurse on the
ward is being asked to prepare
materials or blood for the
procedure. The clinic is set to be
the minor operating room with
materials prepared aseptically.
Patients
are
now
being
transported to the hemodialysis
clinic.
Prehemodialysis
assessment is being done.
Considering the vital signs,
including weight. The Surgeon
and Nephrologist are now ready
and insertion is initiated. After it
is successfully done hemodialysis
is being initiated. Ultrafiltration
Goal , Blood Flow Rate, Heparin
Needs or Bolus is being asked
from the Nephrologist. The
patient is now on hemodialysis
being strictly monitored.
