that the bench grinder wheel has been used to grind non-ferrous
metal. (build up of this material can cause imbalance and
possible disintegration. The tool rest is not installed and/or
adjusted to within 1/8" of the grinding or wire wheel.
Transparent, non-shatter eye shields are not mounted over the

in accordance with OPNAVINST 5100.19E. The Safety Officer does
not maintain a safety discrepancy tracker, or maintain
documentation of hazards identified through inspections, surveys
or as reported by individuals. Documentation should contain at
a minimum;
(1) Date, time, location, and description of hazard
(2) Risk assessment codes assigned by the safety officer
(3) Recommended corrective action to control or eliminate hazard
(4) Action taken to implement that correction
(5) Verification and date hazard corrected/eliminated. REF:

(b) (S)(B2A0) Not all flammables, hazardous
materials, and chemicals are appropriately labeled as to their
hazards. Bottles are missing secondary labels. (B2B1) Not all
Flammables are stowed in a manner that minimizes fire hazards.
(B2C1) Not all Stowage locations for HM are posted with
appropriate caution signs (NSN 9905-01-342-4851 for 10x7 inch or
9905-01-342-4859 for 3x5 inch) and not all HM locker are labeled

(S)(R) (A5A3) The red devil blowers electrical

fittings are not lock-wired with the required tag attached. The
blowers require a "warning" or "caution" sign attached
concerning operation in an explosive atmosphere. The safety
screens are not installed on the red devil blower, and they are
not in good repair. The red devil blowers are not electrically

the range guard system installed on the submarine, specifically
the ventilation nozzles above the grill are not being checked.
Recommend removing overhead panel to access ventilation cover
and perform appropriate PMS on these nozzles. There is not at
least 3" of free travel between the cable tube and the ends of

(c) (S)(R) (B1B8) PKP extinguisher PMS is not being
conducted IAW applicable MRC's. Not all PKP extinguisher fill
cap seat and gaskets have a light coat of silicone compound
applied. The gaskets are worn and pitting. Recommend conducting
all associated PMS on all PKP extinguishers and replacing all
gaskets. Not all PKP extinguisher brackets contain all rubber

LBS. 9 OZS.-28 LBS. 4 OZS.) Not all AFFF extinguishers and
brackets are firmly mounted. Not all AFFF brackets contain all
