SPECIFIC ROLES AND RESPONSIBILITIES:

Before entering the floor: Check for par level equipment/ Update SharePoint equipment list Stock specialty equipment (Walkers, Canes, Slings, etc.) Grab 1-2 pairs of socks from clean utility room Contact Physical Therapist via Voalte (ask if they need anything and what the plan will be) Before entering patients room: Maintain infection control/ Perform proper hygiene techniques (Cal stat, Gloves, Yellow Gown) Retrieve specialty equipment if instructed by Physical Therapist (Quick Assist, Arjo, PVC walker, Portable heart monitor, etc.) Retrieve/ set up patient supplies if instructed by Physical Therapist (Walker, Oxygen tank, Reclining chair, Bed sheet, Chuck, Transfer sling, etc.) Upon entering patients room: Introduce yourself (to patient, nurse, doctor, etc.)as well as your role/plan & answer questions within your qualifications (refer staff to Physical Therapist/Supervisor if necessary) Prepare patient for therapy (remove Pneumos, Prevlons, Blankets/Sheets, Pillows, Restraints, Tube feeds, etc.) o Move tray table out of the way o Move bedside or recliner chair into position (ventilator side if present) o Place sheet and green pad on chair o Move additional chairs/commode out of the way o Unplug IV pole from wall o Untangle IV lines from other pieces of equipment o Untangle lines (oxygen, BP cuff, cardiac monitor, suction tube, call bell) to promote mobility Check for additional tubes (foley catheter, rectal tube, chest tube, Vac) and move in accordance with mobility plan ie: Pin drains to gown) Record patient data (Hemodynamics, Time-in, Vent/Trach/O2 settings, etc.) if not connected may have to reconnect to obtain data During treatment/ transfer: Follow Physical Therapists step by step instruction & ask for clarification/assistance if uncertain Continue to mind tubes (NG, Rectal, Catheter), lines, IVs, vacs/drains, equipment cords (ECG, PSO2, BP) Record patient data (Hemodynamics, Time-in, Vent/Trach/O2 settings, etc.) Post Transfers: Reconnect (if disconnected) equipment cords (ECG, PSO2, BP)

Replace additional equipment so they are secure and out of the way (IV poles, rectal tubes, chest tubes, urinary catheters, drains) Replace attached equipment (pneumo boots make sure machine is on, prevalon boots), take lift straps out from under legs, Put bed in lowest position with side rails up o Confirm with therapist if bed/chair alarm and/or restraints need to be on

Prior to Leaving Room: Assure patient has call bed, tray table by side so that they can reach everything. Ask if they have any other needs water (check with RN to see if they can have), blankets
