EXAMINATION
Please be informed of the following schedule of act;vities in relation to the 2014-
2015 Accreditation Examination (AE):

Deadline of submission of application
Transmittal of test materials from OED to the regional
campuses c/o the Campus Director. The test materials will
be sent through courier or by special arrangements with the
OED.
Orientation of proctors by the Chief Examiners
Administration of the Accredilation Examinat;on, half day
only. (Please refer to the Test Administration Guide for the
su8gested schedule and the time allotted for each type of
test. Deviation from the said schedule is allowed only with
prior clearance from the Office ofthe Executive Director.)
Transmittal of test materials back to the PSHS System-OED,
together with all other test paraphernalia (e.9.
accountability forms, test administration
guides, etc.) and
application forms
ln order to ensure smooth conduct as well as integrity of the test activity, the
following guidelines shall be followed in relation to the conduct of the SY 2014-2015
Accreditation Examination:

Executive Director
1. The Campus Director has over-all responsibility in the conduct of the PSHSS
Accreditation Examinations. The CISD/SSD Chief will act as the Chief Examiner to be
assisted by the Guidance Counselor as Assistant Chief Examiner. Chief Examiners
may recommend teachers or staff to act as Proctors to administer the accreditation
examination, as well as to orient them on their duties as well. ln each campus, the
assignments of faculty and staff to different functions in relation to the conduct of
the AE shall be disseminated through an Administrative Order issued by the Campus
Director.
2. Test Administrators
(i.e.
Chief Examiner/Proctor) have the following specific duties
and responsibilities:
a. The chief Examiners:
i. Select the proctors who will be assigned to each testing room.
ii. See to the safekeeping of test materials when these arrive by courier, and
until they are transmitted back to the PSHS System Admissions Office
after the conduct of the AE.
iii. Orient the proctors on the conduct ofthe AE.
iv. Distribute to the proctors the test materials at the start of the day of the
tests.
v. Transmit the test materials back to the OED immediately after the test. ln
no instance shallthe test materials be handcarried by person/s or groups
who are not PSHS personnel.
b. The Proctorsi
i. Receive the materials for the tests which are scheduled to be
administered, from the Chief Examiner.
ii. Report to the testing room not later than 7:15 AIM, and start the
examination at exactly 7:30 AM.
iii. Check the attendance ofthe students and assign them seats.
iv. Onlythepencil anderasershall be allowed on the desks. cellphonesshall
be switched off and have to be deposited inside the student's bags, which
will be placed in a designated area within the testing room.
v. Check that the answer sheets are properly filled up by the students.
vi. See to it that the room is free from noise and interruption while the test
is going on.
vii. Make sure that there is NO CHEATING that will take place.
viii. Proctors should not do anything other than see to the proper conduct of
the exam (e.g. no reading of books, chatting with co-prodors, surfing the
net, use of earphones and cellphones).
3. The time allotment for each test is as follows:
a. Mathernatics
-
90 minutes or 1 HOUR & 30 MINUTES
b. Science
-
75 minutes or 1 HOUR & 15 MINUTES
c. English
-
60 minutes or 1 HOUR
4. On the use ofcalculators and scratch
paper:
a. The use of calculator is NoT ALLOWED for MATHEMATICS'
b. Scratch
papers will no longer be included in the test
packages
c. Students can use the margins ofthe test booklets or any vacant space in the booklet for
their scaatch work.
