Job Title: Skills for Daily Living Support Worker


Salary:

13,676 per annum

35 hours per week

To contribute to the multi-agency approach in ensuring
that the vulnerable adults are enabled and empowered
to sustain independent living in their own homes.

Job Purpose: To support vulnerable adults to develop and maintain
daily living skills to maximise their independence and
make informed choices thus preventing or delaying the
need for more intensive care and support services.

Reporting to: Move on Service Manager

Key Responsibilities and Accountabilities

To undertake full Daily Life Skills Support needs assessments offering advice and
information in accordance with the remit of the service.

To agree a support plan to maximise outcomes and individuals independence.

To assist in the provision of appropriate housing, advice and information to individuals,
families and carers to support them in making informed choices about their housing
preferences.

To undertake initial and on-going assessment and consultation with customers, line
managers and other agencies. Individual support plan devising, monitoring, evaluation
and supporting documentation.

Reporting any concerns (e.g. threats, self-harm, abuse, illness) to the line manager.

To observe confidentiality at all times.

Increase peoples confidence to remain safe at home by addressing safety and security
of vulnerable persons and their home environment.

Promote and uphold the Midland Heart policy on equal opportunities.


To assist in times of staff shortages at any time.

Undertake other duties commensurate with the grading of the post as may be required.

Adhere to Midland Heart Policies & Procedures.

To be responsible for the health, safety and welfare of yourself and others at work and
to undertake the health and safety duties outlined in the Midland Heart Health and
Safety Policy commensurate with this position.

Education, qualifications and training
Relevant NVQ 2 or equivalent or willing to work towards.
Experience
Must be able to undertake needs assessments and to agree a
support plan to maximize outcomes.

Be able to provide practical assistance and support vulnerable
people with activities of daily living .

Have experience in promoting individuals independence and
encouraging and supporting the use of community facilities.

Experience of working with vulnerable adults and people with
disabilities.

Knowledge
Local safeguarding policy.
Have an understanding of the principles of choice, privacy,
dignity and respect.

Skills
Able to communicate and relate with vulnerable adults and
their family & carers.

Able to transfer observations and actions into accurate, legible
written reports.

Able to use various IT systems with good keyboard skills.
Able to communicate effectively orally and in writing.
Behaviours
Able to work on own initiative & as part of a team.
Flexible working hours if customers require support outside
office hours.
