CONTRACT OF SERVICE

Be it known to all concerned that JENEFER P. LAYOGAN of
Legleg, Palina, Kibungan As the first Party, of
LEGLEG BARRIO SCHOOL LEGLEG, PALINA, KIBUNGAN
School School Address
Kibungan and Benguet Wangal, La Trinidad, Benguet
District Division Division Address

Second Party, representing the Department of Education do hereby agree to the following:
A. Responsibilities of the First Party

1. Teach / handle Kindergarten Volunteer Program for a period of three (3) hours per class for 40
weeks.
2. Undertake necessary preparations before teaching to ensure quality experiences and care for
the Kindergarten children.
3. Adhere to the set kindergarten standards by the department.
4. Submit to the school administrator needed reports which include, but not limited to enrollment
ECCD checklist and others.

B. Responsibility of the Second Party- Department of Education
1. Supervise the teachers of KVP classes to ensure that classes are implemented according to the
standard set.
2. Provide the necessary support to the kindergarten teacher.
3. Certifies attendance and payment of teacher's salaries.
4. Maintain active linkage with the LGU and other interested GO'S and NGO's to draw-in support to
the kindergarten education.

For and in consideration of this contract, the Division shall pay:
1. Three Thousand Pesos (PhP 3, 000.00) per month for One (1) class handled with at least 11
pupils per class /session. In case of one (1) class with 6-10 enrolment, a teacher will receive Two
Thousand Pesos (PhP 2, 000.00) per month only.

IN WITNESS WHEREOF, the parties hereto have signed this Contract of Services
