SAFETY TOPIC WEEK 17, 2007


ACCIDENT / INCIDENT INVESTIGATION & REPORTING ACCIDENT / INCIDENT INVESTIGATION & REPORTING
The investigation shall be based on ROOT CAUSE ANALYSIS, and shall report
incident causation, responsibility or fault, corrective actions, and/or lesson
learned.TSK shall disseminate the lesson learned from incident investigations to its
employees at site without delay.
For all incidents and near misses, with the potential for fatality, serious injury or
significant environmental or material damage SUB. CONT. should give an
immediate verbal report followed by 0-60-120 report within 02 hours and final
report within 07 days to TSK.
TSK should provide an immediate verbal report following all incidents except FIRST
Aid Cases. And a written preliminary report is to be forwarded to OWNER within 02
hours of the incident. The final report will be submitted to OWNER within 07 days of
the incident. The investigation team composition will be decided by TSK.

Every Incident are reported promptly.
Key data is provided (injuries, persons involved, circumstances, outcomes,
consequences, EHS management system failure).
Incidents are investigated appropriately.
Recommendations resulting from investigations are implemented.

FIRST AID - Injury/illness requires minimal medical assistance.
DOCTOR VISIT-No treatment only advise or counseling.
MEDICAL TREATMENT- Recordable and requires medical treatment.
RESTRICTED WORK CASE- An injury/illness that involves one or
more days of restricted work or a job transfer determined by physician.
LOST WORKDAY CASE- An injury/illness that restrict employee to report to
work for his next scheduled shift as determined by a physician.
FATALITY-Self explanatory.
ENVIRONMENTAL INCIDENT-Reportable environmental incident, or a spill
or loss of containment.
MATERIAL DAMAGE-Material damage having a cost of $5,000 in any on incident.
NEARMISS Incident having the potential to cause serious injury/fatality/
disability/environmental damage or material loss.
Occurrence Sub Contractor Verbal information immediately (except First
Aid Case), Written notification within 02 Hours (0-60-120 minutes report) and
final report within 07 days to TSK.TSK Management Verbal information
immediately, Notification within 02 hours (0-60-120 minutes report), detailed
report within 7 days to OWNER
