Checklist for H-1B Petition

Instructions for the Hiring Department

Please complete the H-1B Request Form and collect all the supporting documents listed below. Submit all documents to
Loveness Schafer, International Services Office, 101 Hatcher Hall. The Immigration processing time for an H-1B varies
from two to four months. Call (578-3191) or e- mail (lschaf2@lsu.edu) if you have questions.

1. Approved copy of HRS print screen.
2. Export Control Attestation (sent by dept to Office of Intellectual Property, copy to Loveness Schafer).
3. Request Form (complete on-line on our website).
4. Departmental Letter on behalf of Employee (Sample on ISO web site).
5. Copy of job offer letter and contract if a tenure-track position.
6. Copy of full job description including duties, required experience, and education.
7. Statement of employment history in the U.S. within the past 6 years, including dates, employer, job title,
and visa classifications.
8. Copy of last 2 pay stubs, if employed in the United States in H-1B status.
9. Summarized statement of prior relevant work experience.

Number 9 to 12 apply only in the case of initial (new) H-1Bs, not extensions:
10. Copies of all Forms I-20 issued if H-1B beneficiary is in F-1 status now.
11. Copy of Employment Authorization Card if employee has/had Optional Practical Training (OPT).
12. Copies of all DS-2019s and/or any IAP-66s if H-1B beneficiary is or has ever been in J-1 status.
13. Copy of USCIS waiver or evidence of fulfillment of two year foreign residence requirement if beneficiary
is subject to the requirement. Department of State letter of recommendation for waiver may be submitted
if application is still pending with USCIS.
14. Copy of transcript
15. Copy of current I-797 if request is for renewal/extension or transfer of H-1B.
16. Copy of resume / CV
17. Copy of highest diploma and credential evaluation if degree is not from the U.S.
Many U.S. private companies evaluate diplomas; for example, Global Credential Evaluators; see:
gcevaluators.com. If degree is from a U.S. institution, please provide the address of the institution.
18. Copy of Form I-94 (front and back)
19. Copy of passport, including visa page
20. FEES:
i. $500 Fraud Prevention and Detection Fee: applicable only for an INITIAL H-1B.
ii. $325 Filing Fee: required for every petition.
iii. $1,225 Premium Process fee: applicable if requesting expedite service.
Immigration processes premium service petition within 15 calendar days.
Make all money orders or checks payable to Department of Homeland Security.
Each payment must have a separate check or money order in the exact amount.
All checks and money orders must be drawn on a bank or other institution located in the United States.

All fees listed above are the sponsoring departments responsibility.
Exception: Premium processing fees which are necessary for personal reasons (such as personal travel not
required for the job) are the employees responsibility.

Dependents

If dependents are in the U.S. (petition not required if they are abroad) please also submit:

20. Form I-539 prepared and signed by the spouse for a change of status or extension of status to coincide
with the petition dates for the H-1B beneficiary. (Find Form I-539 on Immigration website at:
www.uscis.gov). Sign form in blue ink.
21. Copies of dependents I-94 cards (front and back)
22. Copies of dependents passports, including visa pages
23. Copy of marriage certificate (for Dependent spouse); copy of birth certificate (for dependent child).
24. A filing fee of $290 for family members already in the U.S. regardless of number.
Filing fees for dependents of the H-1B employee are the employees responsibility.

Documents not in English

Any document not in English must be translated and accompanied by a translators statement as follows:

I, __________________________________, hereby certify that I am competent to
translate from ____________________language into English and that the attached is an
accurate translation of the original document.
