In order to secure a space for the 2010-2011 school year, this form must be MAILED to:
Nurturing Knowledge School: 6203 Phinney Ave N - Seattle, WA 98103 by December 19, 2009. No
money is due with this form. Available spaces will be filled as completed forms are received. (The earlier
your forms are received the greater the likelihood of confirming your preferred schedule.) If your plans
change we ask that you let us know as soon as possible, so we can accommodate another waiting family.
Deposits will be forfeited if withdrawal from the 2010-2011 school year occurs after 5/15/09.

Note: There is a 10% discount on sibling tuition rates, this discount only applies to sibling’s and not to the first child enrolled. A
$350 deposit is due in February for new siblings starting in September of the 2010-2011 school year.

2010/ 2011 Enrollment Agreement

● Tuition is invoiced via e-mail by the 25th of the month and tuition is due on the 1st of the following month. Payments received
after the 5th of the month will incur a $25.00 late fee which will be included on your next month’s statement. If you do not
receive an invoice tuition is still due and payable by the first of the month.
● There is no credit for absences and days that the school is closed.
● Schedule Changes: Schedule changes are made on a space availability basis. Schedules can only be changed once per month.
To change your child’s schedule a “Schedule Change Form” must be signed and received by the 15th of the month in order to
adjust billing amounts for the next month’s tuition.
• Withdrawal/Deposit Policy: To withdraw your child from the program a 45 day written notice is required. A “Withdraw
Form” must be signed and received by the 15th of the month in order to adjust billing and apply your deposit to your last month
of tuition. If proper notice is NOT given, tuition will accrue for 30 days and your deposit will be forfeited.
• Deposit Policy: Deposits are applied to your last month tuition when proper notice is given. (see withdrawal deposit policy above)
• A $350 deposit is required for each child enrolled
• A Annual Registration Fee of $75.00 per family is due each September and is Non-Refundable.
• School Calendar Year: Bryant, Phinney and Woodland Park operate from September through Mid June. Monthly tuition is
based on the number of school days in a year divided by 10 months. Full payment is charged for each month. (i.e. although
December and June are shorter months full tuition is still due)
• School Calendar Year: Greenwood Campus is a year round program, September through August.
• The signature page of the Parent Handbook and the “Family information form” must be on filed out and signed prior to your
child’s first day.
• Deposits will be forfeited if withdrawal from the 2010-2011 school year occurs after 5/15/09.
I agree to adhere to the stated policies and procedures of Nurturing Knowledge School as stated here and in the Parent Handbook. I
give my child permission to participate fully in this program. My signature also states that I am the party responsible for all payments.
