1.1 Product Name
The device presented in this Clinical Risk/Benefit Analysis (CBRA) is XXX YYY Paclitaxel-Eluting Coronary Stent System (hereafter referred to as the ZZZ System).
1.2 Device Classification
Europe
According to the Medical Device Directive (MDD) 93/42/EEC, and the associated Guidelines Relating to the Demarcation Between Directive 93/42/EEC Relating to Medical Devices and Directive 65/65/EEC Relating to Medicinal Products and Related Directives, this product is classified as a Class III device. This classification is based upon the fact that the action of the chemical agent, paclitaxel, is ancillary to the mechanical support the coronary stent provides in the treatment of coronary artery disease. The principal mechanism of action of this product is mechanical aided by the proposed action of the paclitaxel inhibiting the neointimal hyperplastic response caused by the stent implantation. The following are the rules that apply to the classification of this device:
Class III, MDD Annex IX per Rule 6 (Catheter, Angioplasty, Balloon Dilatation). The balloon catheter delivery system is a surgically invasive device for transient use intended to deliver the stent to the afflicted vasculature.
Class III, MDD Annex IX per Rule 8 (Prosthesis, internal, Stent, Cardiovascular). The balloon expandable stent is an implantable device to be used in direct contact with the central circulatory system.
Class III, MDD Annex IX per Rule 13 (Special Rule) The device incorporates, as an integral part, paclitaxel which, if used separately, can be considered to be a medicinal product, as defined in Article 1 of Directive 65/65EEC, and which is liable to act on the human body when action ancillary to that of the device.

DISPOSITIVOS
1.1 Nombre del dispositivo
El dispositivo que se presenta en este Anlisis Clnico del Beneficio y del Riesgo (CBRA, por sus siglas ingls) es XXX YYY Stent Coronario Liberador de Paclitaxel (en adelante denominado Sistema ZZZ).
1.2 Clasificacin del dispositivo
Europa
Segn la Directiva de Productos Sanitarios (MDD, segn sus siglas en ingls) 93/42/CEE y las correspondientes directrices relativas a la demarcacin entre la Directiva 93/42/CEE sobre productos sanitarios y la Directiva 65/65/CEE relativa a los medicamentos y directivas conexas, este producto se clasifica como producto clase III. Esta clasificacin se basa en el hecho de que la accin del agente qumico, paclitaxel, el cual es auxiliar al soporte mecnico que proporciona el stent coronario en el tratamiento de la cardiopata isqumica. El principal mecanismo de accin de este producto es suministrar un soporte mecnico a la accin propuesta por paclitaxel que inhibe la reaccin hiperplsica neointimal provocada por la implantacin del stent. A continuacin figuran las reglas que se aplican a la clasificacin de este dispositivo:
Clase III, Directiva de Productos Sanitarios (MDD), Anexo IX, Regla 6 (catter, angioplastia, dilatacin del globo). El sistema de suministro del catter con globo es un dispositivo quirrgicamente invasivo para uso transitorio con la funcin de colocar el stent en la vasculatura afectada.
Clase III, Directiva de Productos Sanitarios (MDD), Anexo IX, Regla 8 (prtesis, interno, stent, cardiovascular). El stent expandible por baln es un dispositivo implantable para utilizarse en contacto directo con el sistema circulatorio central.
Clase III, Directiva de Productos Sanitarios (MDD), Anexo IX, Regla 13 (consideracin especial). El dispositivo incluye como parte integral, paclitaxel, el cual si se usa por separado se puede considerar un medicamento, como se defina en el Artculo 1 de la Directiva 65/65/CEE, y que es responsable de actuar sobre el cuerpo humano cuando una accin accesoria a la del dispositivo.

-Based on the FDA response dated May 16, 2001 to XXXs Request for Designation dated March 30, 2001, the product is subject to premarket review and approval under the medical device provisions of the Federal Food, Drug, and Cosmetic Act, and specifically is subject to premarket application (PMA) submission requirements to provide reasonable assurance of its safety and effectiveness.
Although the product will be subject to PMA and other medical device regulatory requirements, the paclitaxel component of the combination product may be subject to certain drug requirements, including human drug current good manufacturing practices requirements.
1.3.2 Delivery Catheter
The ZZZ System utilizes the ZZZ delivery catheter that is used as part of the ZZZ Coronary Stent System.
The distal section of the catheter includes dual, coaxial lumens. The outer lumen is used for inflation of the balloon, which results in deployment and expansion of the stent. The inner lumen permits the use of guide wires (= 0.36mm/0.014 inches) to facilitate advancement of the catheter through the lesion to be stented. The proximal section of the catheter is a single lumen, polytetrafluoroetheylene (PTFE) coated, stainless steel hypotube with a single luer port for inflation/deflation of the balloon. The catheter includes a tapered tip to facilitate advancement of the catheter through the lesion.
The delivery catheter is low profile in design, making it compatible with 5 French (F) (for 2.25-4.0mm) and 6F (for 4.5-5.0mm) guiding catheters (minimum lumen internal diameter [ID] of 1.47mm/0.058 inches and 1.68/0.066 inches, respectively).
Deployment of the stent to nominal size is completed with balloon expansion pressures equal to 9 atmospheres (atm). The delivery catheter is also designed for post-stent dilation using pressures less than or equal to the rated burst pressure: 18 atm for balloon diameters less than 4.0 mm and 16 atm for balloon diameters of 4.5-5.0 mm.
The stent is centered on the balloon between two (2) radiopaque marker bands to aid in positioning the system during the procedure. The balloon extends beyond the stent ends to ensure full expansion of the stent during deployment while minimizing the amount of balloon outside the stent region. The distal shaft (excluding the balloon/stent region) of the delivery catheter is coated with Bioslide hydrophilic coating to enhance device tracking performance.
The implementation of new bond processing technologies (laser welding), a flexible, tapered tip at the distal end of the catheter (bumper tip), and a PEBAX distal shaft is used (g), (21 USC 353 g):

-Basado en la respuesta de la FDA, del 16 de mayo de 2001, a la solicitud de designacin de XXX, el producto es sujeto a evaluacin y autorizacin previa a la introduccin al mercado segn la provisin de dispositivos mdicos de la Ley Federal de Alimentos, Medicamentos y Cosmticos, y especficamente sujeto a los requisitos de la solicitud para la aprobacin del pre-mercado (PMA, segn sus siglas en ingls) para proporcionar una garanta razonable de seguridad y eficacia.
Aunque el producto ser sujeto a la PMA y otros requisitos reglamentarios de dispositivos mdicos, el componente paclitaxel del producto combinado puede ser sujeto a ciertos requisitos farmacuticos, incluyendo las actuales requerimientos de buenas prcticas de manufactura para medicamentos de uso humano.
1.3.2 Catter de colocacin
El Sistema ZZZ utiliza el catter de colocacin ZZZ que forma parte del Sistema de Stent Coronario ZZZ.
La parte distal del catter tiene un diseo de doble lumen coaxial. El lumen exterior se utiliza para el inflado del baln, resultando en el despliegue y la expansin del stent. El lumen interior permite el uso de cables gua (0,36 mm / 0,014 pulgadas) para facilitar el avance del catter a travs de la lesin a ser tratada con el stent. La parte proximal del catter se compone de un lumen recubierto de politetrafluoroetileno (PTFE, por sus siglas en ingls) de acero inoxidable compuesto por un hipotubo con un puerto luer encargado de coordinar la inflacin y la deflacin del baln. El catter tiene una punta cnica para facilitar su progresin a travs de la lesin.
El catter de colocacin presenta un perfil bajo (menor grosor) y es compatible con 5 French (F) (para 2,25-4,0 mm) y 6F (para 4,5-5,0 mm) catteres guas (mnimo dimetro del lumen interno [ID, por sus siglas en ingls] de 1,47 mm / 0,058 pulgadas y 1,68 / 0,066, respectivamente).
El despliegue del stent a tamao nominal se logra con la expansin del baln con presiones de 9 atmsferas (atm). El catter de colocacin tambin est diseado para post dilatacin del stent utilizando presiones menores o iguales a la presin de ruptura: 18 atm para globos con un dimetro menos de 4,0 mm y 16 atm para globos con un dimetro de 4,5-5,0 mm.

On the next page Table 2 outlines the characteristics of the ZZZ Monorail and Over-the-Wire delivery systems. El stent se centra en el globo entre dos (2) bandas indicadoras radiopacas para ayudar en el posicionamiento del sistema durante el procedimiento. El globo se extiende fuera de los extremos del stent para asegurar la expansin completa del stent durante el despliegue mientras reduce la cantidad del globo fuera de la regin del stent. La segunda generacin del catter de colocacin ZZZ se aprovecha de la implementacin de nuevas tecnologas de elaboracin (soldadura por lser), una punta flexible y cnica en el extremo distal del catter (punta parachoques), y un eje distal PEBAX. Un eje corto Grilamid pasa del hipotubo al eje distal PEBAX.
