Innovation and Entrepreneurship Certificate

Please complete this form and send it electronically, along with a copy of your resume, to Hayley Young
(hayley.e.young@duke.edu). Your proposal will be carefully reviewed by the Advising Committee, and
you will receive notice as soon as possible about whether or not you have been accepted into the
certificate program. No coursework or experiences will count toward the certificate until your proposal
has been officially approved. The deadline to submit your proposal is the add/drop day of the fall of
your junior year.

Advising is a large component of the certificate program. Significant guidance will be available to you as
you progress through the certificate, to help you refine your thinking and ensure a meaningful
experience.
