Shoulder pain radiating to the neck, shoulder pain
leading to stiffness of the neck with inability to turn
the head, neck pain.
COMMENTARY
Tianchuang SI-16 is one of ten points listed in Chapter 2 of
the Spiritual Pivot that have come to be known as Window
of Heaven points. As a group, these ten points have the
action of regulating rebellious and chaotic (inversion) qi.
Tianchuang SI-16 illustrates many of the characteristic
actions of these points in its ability to i. treat goitre and
throat disorders, ii. treat headache and heat and swelling
of the face and cheek, iii. treat disorders of the sense
organs - in this case the ears, iv. treat sudden onset of
disorders - in this case loss of voice. In addition, like two
other points from this group (Tianzhu BL-10 and Tianfu
LU-3) Tianchuang SI-16 treats disorders characterised in
the classical texts as seeing or communicating with ghosts
and therefore attributed to some form of demonic posses-
sion. In terms of modern medicine these indications refer
to various forms of severe mental disorder including
schizophrenia. For a fuller discussion of the Window of

Sun Si-miao, the famous 7th century physician, particu-
larly emphasised the use of moxibustion (fifty cones) at
this point for loss of speech and hemiplegia.
COMBINATIONS
Ear pain, deafness and tinnitus: Tianchuang SI-16,
Yangxi L.I.-5, Guanchong SJ-1, Yemen SJ-2 and
Zhongzhu SJ-3 (Thousand Ducat Formulas).
Deafness and tinnitus: Tianchuang SI-16 and
Waiguan SJ-5 (Supplementing Life).
Throat pain: Tianchuang SI-16, Fengfu DU-16 and
Laogong P-8 (Thousand Ducat Formulas).
Goitre: Tianchuang SI-16 and Naohui SJ-13 (System-
atic Classic).
Sudden loss of voice: Tianchuang SI-16, Zhigou SJ-6,
Futu L.I.-18, Qubin GB-7 and Lingdao HE-4 (Thou-
sand Ducat Formulas).
Sudden loss of voice with lockjaw: Tianchuang SI-16,
Lingdao HE-4 and Tiantu REN-22 (Supplementing Life).
Lockjaw: Tianchuang SI-16 and Yifeng SJ-17 (Supple-
menting Life).
Heat sensation of the skin of the face: Tianchuang
SI-16 and Tiantu REN-22 (Supplementing Life).

Point of the Window of Heaven
LOCATION
On the posterior border of the sternocleidomastoid mus-
cle, level with the laryngeal prominence.
LOCATION NOTE
i. Palpation of the posterior border of the sternocleidomas-
toid muscle is made easier if the patient turns their head
away from the side to be needled, whilst you apply resist-
ance at the chin; ii. In females the laryngeal prominence is
not as pronounced as in males. If it is indistinct, palpate the
depression formed by the lower border of the hyoid bone
and the upper border of the thyroid cartilage at the midline.
The laryngeal prominence lies just below this.
NEEDLING
Perpendicular insertion 0.5 to 0.8 cun.
ACTIONS
Benefits the ears, throat and voice
Regulates qi and calms the spirit
Activates the channel, alleviates pain and clears heat
INDICATIONS
Deafness, tinnitus, ear pain.
Sudden loss of voice, sudden loss of voice following
windstroke, hoarseness due to cold in the throat,
lockjaw, clenched teeth from windstroke, headache.
Throat pain, goitre, swelling and pain of the cheek,
heat sensation of the skin of the face, urticaria.
Manic ghost talk, mania-depression.
