Chapter 1
PROJECT OVERVIEW
Background of the Study
The world is advancing, and new technologies are being
introduced almost on a weekly basis. These technologies are
changing lives and improving businesses. From social media
to blogs, to entertainment sites, information is
disseminated to millions of people the world over. For this
to be achieved, most companies need to allot a large amount
of storage space, in order to meet the needs of ever-
growing information-hungry generation.
Information technology has affected the way healthcare
systems acquire, store, access and communicate medical
information. These developments offer significant benefits
to patients and healthcare providers, but they give rise to
ethical and legal challenges in the protection of patient
privacy and confidentiality (Jing Jin, 2003)
The ability to provide a complete and reliable patient
dental record is considered as a vital factor in the
continuing efforts towards a more effective, patient-
centered dental care. Current dental practices, which make
use of pictographic symbols and codes, differ among
practicing dentists and these may lead to miscommunication
and visual interactivity.(Kachymalay,Aziz,Hussain, 2010).

Doctors are benefitting immensely from the drive
towards electronic medical records. With one touch of a
button, doctors can access all the care a patient has
received and can figure out possible illness. (Hendricks,
2013)
The product of human creativity and with the
continuous development of technology, information became so
much more rigid in handling and organizing it, so much
information that gathering it and organizing it manually is
impossible which caused some companies to have poor
performance in terms of handling records particularly in
the field of health because In the operational management
of health and medical records. When it comes to this
subject should be noted that the amount of data collected
in health facilities is very large. Provision of medical
services is very specific and complex work whose basic
feature is the number and variety of data and information.
Information System contributed and eliminated the
above mentioned problems. IS became very beneficial to the
hospital/clinic staffs by eliminating paper-based
administration and transformed computer processing of data
is obtained at least a threefold increase in efficiency and
huge material savings in any sphere of modern business. The
appearance of Internet and spread of computer use in the

eighties and nineties of the last century has led to major
changes in dentistry. (Cribbs, 2010)
There are some existing Web-Based information systems
that were created for Dental Clinics. One of them is Curve
Dental; this Information software is a web-based system
intended for setting an appointment and sending a reminder
to patients via email or text message and has a two-way
data integration which allows the practices Demandforce
and Curve systems to sync data in real time. When a patient
confirms an appointment through Demandforce, the Curve
management system will automatically update, leaving front
desk staff and management free to focus on what matters the
most; patient satisfaction.(Tynan, 2011)
Another is OACIS clinical Information System, Open
Architecture Clinical Information System (OACIS) uses a
built-in connectivity engine to gather disparate patient
data from across different systems and transport it into
one central repository to provide a secure, clinician-
friendly tool for documenting patient information and
order-entry management. Datacon is a fully integrated
clinical charting, billing, and patient scheduling system
that provides a host of features for dental practices. This
completely scalable system is a great choice for growing
practices, and can meet the needs of a single-doctor

practice just as easily as large practices with multiple
locations but the primary focus of this system is the
Charting aspect.(Sam, 2005)
These existing information systems have outstanding
and unique features but were only intended to their primary
feature namely for Curve Dental, it was intended for
appointments, OACIS focused on records and documents of the
patients, and Datacon for Charting purposes.
A study is conducted in Genesis Dental Clinic located
in Baguio City and had come up with a conclusion that most
dental clinics here in Baguio (if not all of them) are
running with a manual type of database management for
patient records. The most common problems that are present
in this kind of environment are limited file storage, time-
consuming searching patient record and also data redundancy
(Benito, 2012).
In order to have a good web-based dental information
system, the clinic that chooses to create such system
should have a good internet connection to maintain the
usefulness of the system (Dupais, 2012), administrators
should have an ample training in using the system with a
system manual in hand, the clinic should also have a good
internet connection to retrieve data the fastest way
possible. Another feature that would be very helpful will

be the online deploying of patient diagnosis results, this
can be attained successfully if the doctors would just use
the internet and mail their customers the results of the
diagnosis for the patients not to wait for a longer time to
see his or her diagnosis results.
An e-Medical RMS for Payno Medical Clinic is a web-
based environment which is systematic documentation of
patients medical history, diagnosis and results of
laboratory examination and care. It is a way to organize
medical records for an easier access of patient
information. In order to create such system, the authors
found out that the traditional system follows Subjective,
Objective, Assessment and Plans (SOAP) method of doctors
and some problems were found in this type of method
including costly paper-based medical records, unorganized
medical records, recording and analyzing medical records
and findings are based in the patients symptoms, difficulty
in generating reports and lack of computer security.
Features of the e-Medical RMS include database management,
search function, user account management, reports
generation, security measures in accessing contents (esp.
Patient Records), medicine information and message
bulletin. E-Medical RMS should be utilized to lessen the
workloads of the doctors of Payno Medical Clinic, save

time, paperless environment and a big plus in the patients
convenience. (Andres, 2012)
The researchers aim to study the processes,
transaction and requirements of the dental clinic, as a
requisite to developing and designing a web-based
information system for the dental clinic. In due process,
the researchers plan to solve the flaws and faults that are
detected in the above-mention activities, and suggested
better alternatives and solutions.
Bridging the gap between traditional record management
system and a modernized information system, would also be a
way by which staff of the dental clinic would familiarize
themselves with information technology.
Company Profile
Dr. Luz Lapea is a graduate of Bachelor of Science in
Zoology at Far Eastern University Manila in 1978 then
became a certified Dentist at the University of Baguio in
the year 1987. In the year 2004 the Dental clinic named
Dental Care Clinic was established and started to
accommodate 6-10 patients a day with almost 60-70 a week
and 180-300 a month. The clinic has 2 dentists, 1 secretary
with 2 working computers on hand with a good internet
connection inside the clinic.
After years of contemplating together with her daughter

as her co-dentist and her secretary, the clinic was
successfully rendering dental care for every patient that
visits on every regular working day.
The vision and aim of the clinic is to provide a world
class dental service and create a place where patients can
have all their dental needs taken care of with their
comprehensive care. The Dentists hold their values of
quality service, care, trust and patient comfort above all
else and offer a thorough and personalized approach to
patient care. Satisfactory service to the patients and to
provide first-quality dental care in a state-of-the art
environment with careful attention to detail and deep
personal respect is always the mission of the clinic.
Purpose of the Study
The studys purpose is to help the dental clinic,
dentists, patients, researchers and future researchers and
it is stated as follows:
To the Dental Clinic. The proposed system will help
the Dental Clinic in their daily operations and ease their
burden in database management of patient records. It will
also help the clinic to access the most accurate data.
To the Dentists. The proposed system will be
beneficial to the dentists in handling or reading through
the information.

To the Patients. The proposed system will serve as a
source of information for the patients about dental health
that can be browsed to an internet browser. It will also
serve as a tool for arranging appointments instead of walk-
ins and text messages that are more tedious. It will also
serve as a data provider of the most up-to-date information
about dental health.
To the Researchers. This study served as a pathway of
the researchers to the real world applications of skills
and competencies in systems development. This study was a
stepping stone in engaging with real-world circumstances
with applying the researchers own ideas to create such
system.
To the Future Researchers. This study will be helpful
to the future researchers because web-based information
system is quite a trend in information technology nowadays.
Future researchers can make this study as a guide in making
a good study about web-based information system.
Objectives of the Study
The study aimed to develop a web-based dental
information system for Lapea dental clinic. The objectives
of the researchers are as follows:
1. to identify the processes of the existing dental
information system;

2. to identify problems in the existing dental
information system; and
3. to design and develop a dental record information
system that is better compare to its traditional system.
Scope and Delimitation
The study aimed to develop and design a web-based
Dental Record Management System for Dental Care Clinic. The
study focused on developing a system in a web environment.
The features of this study made the system to be efficient,
flexible to any circumstances, and also convenient to the
users.
The researchers created a Web-Based Dental Information
System for Lapea Dental Clinic. There was a log-in for the
administrator (clinic staff) and for patients. Creating
appointments or patient scheduling feature was also
included. Patients can also fill out the form in any
computer they want for them to be added in the patient
records. Billing of patients, inventory records and
printing of bills, diagnosis and prescriptions will also be
included in the system.
These features were key for allowing the system to
change the structure of the clinics daily operation. Thus,
it helped the clinic to provide the most efficient service
to their patients and achieved their goal in terms of the

provision of dental service. It also saved time and energy
for the clinic staff in managing these data.
The researchers did not include payment and expenses
of the clinic. The study was focused on meeting the above
mentioned features and objectives of Web-based Dental
Record Information System.
Definition of Terms
Data Redundancy. Data redundancy is a condition
created within a database or data storage technology in
which the same piece of data is held in two separate
places.
Hardware. It refers to the physical parts of a
computer and related devices. Internal hardware devices
include motherboards, hard drives, and RAM. External
hardware devices include monitors, keyboards, mice,
printers, and scanners.
Information System. An information system (IS) refers
to a collection of multiple pieces of equipment involved in
the dissemination of information.
Scalable. An attribute that describes the ability of a
process, network, software or organization to grow and
manage increased demand.
Software. It is a general term that describes computer
programs. Related terms such as software programs,

applications, scripts, and instruction sets all fall under
the category of computer software.
Web host. It stores all the pages of your website and
makes them available to computers connected to the
Internet. The domain name, such as "sony.com," is actually
linked to an IP address that points to a specific computer.
Web server. It is a computer system that hosts
websites. It runs Web server software, such as Apache or
Microsoft IIS, which provides access to hosted
webpages over the Internet.

Chapter 2
METHODOLOGY
This chapter discusses the software development
methodology, data gathering techniques, sources of data and
software development tools that will be used to complete
the proposed system.
Software Development Methodology
The researchers chose the Rational Unified Process
(RUP) as a methodology to use as a guide in developing the
proposed system of Web-based Dental Record Information
System. To be more familiar with RUP, it stands for
"Rational Unified Process." RUP is a software development
process from Rational, a division of IBM.

It divides the development process into four distinct
phases that each involves business modeling, analysis and

design, implementation, testing, and deployment. The four
phases are:
Inception. The idea for the project is stated. The
development team determines if the project is worth
pursuing and what resources will be needed.
In this phase, the researchers planned to develop a
web-based dental information system; this is the time where
the researchers started brainstorming. The team discussed
first in conducting meetings with the company and discussed
what are needed. Construction of the system was formulated
in this phase.
Elaboration. In this phase, the project's architecture
and required resources are further evaluated. The
researchers consider possible applications of the software
and costs associated with the development.
In this phase, meetings were conducted and the
researchers had a brainstorming about the software needed
in order to create such system, possible costs and time
frame in completing the proposed system. Costs and time
frame were also in the guidance of the company because they
are the investors of this project.
Construction. In this phase, the researchers would
develop the system. The system would be designed, written,

and tested. The various types of testing are carried out to
ensure the system is properly working.
The researchers designed the system and developed in
order to create a fully functional system. Several tools
were used to create the system using PHP, MySQL for the
systems database, JavaScript, AJAX and CSS which were all
free to download in the internet. Testing between the
developers was also done in this phase.
Transition. The system is released to the clinic.
Final adjustments or updates are made based on feedback
from end users. The RUP development methodology provides a
structured way for companies to envision create software
programs. Since it provides a specific plan for each step
of the development process, it helps prevent resources from
being wasted and reduces unexpected development costs.
In this phase, the transferring of the system from the
researchers and to company was done. The intended users of
the system (the dentist and clinical receptionist) were
oriented on how to use it. When they had tested the system,
their feedbacks and comments were evaluated and changes and
updates were done. Once they became used to the system and
were satisfied with it, it was them deemed the best time to
hand over the system to them and now fully functional to
use.

Data Gathering Techniques
The researchers performed different data gathering
techniques to fulfill the researchers need in building the
proposed Web-based Dental Record Information System and
these are:
Interview. The researchers conducted a series of
interviews with the client for the researchers to identify
their current system and also to identify what they want
their new dental record information system will function.
Observation. The researchers went to the clinic to
observe the clinic in their daily operations in order to
know what transpires, or the processes which are undergone
in each transaction. The researchers own observations were
useful because the researchers had an idea about the mode
of operation within the clinic.
Document Analysis. The researchers studied existing
documents such as forms and reports in order to use the
gathered data in the development of the system.
Sources of Data
Sources of data are responses from Dr. Luz Lapea and
the clinic staff which gathered through interviewing with
the researchers observation to the clinic, the traditional
system was identified including its flaws and problems.
Documents analysis includes some articles in the internet

and existing study about web-based dental information
system and also some forms that are filled out by the
patients and the dentist. These forms include: Patients
Record Form the very first form that is given to the
patient to be filled out in order to proceed on the
consultation and treatment; and Health Questionnaire this
form serves as the basis of medical history of the patient
wherein the questions are all about the experiences of the
patient pertaining to health condition including some past
ailments, operations and the like.
Software Development Tools
The researchers used the following tools in creating,
designing, building and developing the proposed system of
Web-based Dental Record Information System:
PHP. This programming tool was used for its general-
purpose which was server-side scripting language which was
made for Web development to produce a dynamic web site. PHP
was be the only server-side scripting language to be used
by the researchers.
Notepad++. This simple IDE was used for the hard
coding of the proposed system because it was a more
programmer friendly environment and it was easier to use in
debugging web pages.
CSS and JavaScript. These tools made the web pages

more attractive and provided an interaction with the users.
CSS was used for designing webpages of the system for the
purpose of making it more attractive and neat for the sake
of the users. JavaScript was used to provide dynamic
quality of the system.

Chapter 3
TECHNICAL BACKGROUND, RESULTS AND DISCUSSIONS
This chapter discusses the different processes as well
as the problems encountered in Lapea Dental Clinic of Dr.
Luz Lapea. A detailed discussion in the features of Dental
Information System as the proposed system is also stated.
Narrative Description of the Existing System
The researchers conducted an interview and observation
around the clinics daily operation is done to gather all
the necessary information to know the processes involved in
the Lapea Dental Clinic.
Filling out of Patient Forms.The dentist will be
asking the patient necessary personal information needed in
a Patient Form. Then this form will be stored in file
storage.
Storing of Patients Dental Records. In this process,
patient forms, dental records and bills are stored in a
storage drawer and arranged alphabetically.
Attending to Walk-In Patients. Walk-In Patients will
register and schedule their time and date of appointment.
When the dentist is available at the moment, then they can
proceed with consultation and operation.
Manual Scheduling. In this process the registered
patient will reserve the desired time and date of operation

and approved by the dentist who is Dr. Luz Lapea. Schedule
of appointments can also be declined by the dentist and can
give an alternative schedule of consultation or operation.
Patient can now go with the appointment proper once he has
an approved schedule.
Consultation and Operation. In this process, once the
schedule is settled, then the dentist and patient can
proceed to the consultation or operation.
Billing of Patient. In this process when the
consultation or the operation is done. The dentist will
then generate a bill in a form of a receipt. The receipt
then will be forwarded to the staff for the payment of the
bill. Once the bill is paid by the patient, he can go out
of the clinic.
Searching and Update of Patient Records. In this
process, the dentist will then search the patient that he
has recently treated. Once the dentist found the patient
record, he will update the patient record of the recently
treated patient.

Problems Encountered in Lapea Dental Clinic
Data Redundancy. Double entries are made, because
there isnt any mechanism or means by which the system can
check for existing records of patients.
Scheduling Problems. With regards to the schedules,
several problems are encountered like conflicting
schedules, unattended schedules of patients, forgotten
schedules of both the dentist and the patient. The primary
problems of these are that schedules are done and recorded
manually. Dentists and patient tend to forget the exact
date and time of the consultation or operation.
Time Consuming of Searching for Patient Records. This
is another problem occurring in having a physical patient
records and searching for patient records is time consuming
in storage where a bunch of physical patient records is
stored.
Lost and Damaged Records. These are cases whereby
records have already been disposed of, because they may be
outdated, or improperly stored. Records can also be damaged
because they have been tampered with.
Shortage of Patient Forms. There is also a risk of
running out of patient forms at a time when the dentist is
in dire need of it.

Features of the Proposed System
The proposed system would comprise of the following
features:
Administrator and Patient Log-in. This feature can be
the main security feature of the proposed system.
Administrator will be the one to maintain the system.
Administrator can manage patient records and do
appointments. Patients can also log-in for them to fill-up
dental records and do appointments.
Online Reservation. In this feature, patients can have
a desired time and date to have an appointment with the
dentist. But before the appointment to be final,

confirmation of the administrator will be done and the
appointment can be declined or approved by the
administrator.
Reports Generation. This feature lets the dentist
print the reports of daily schedules, and assigned dentists
in Lapea Dental Clinic.
Online Fill-out of Patient Record Form. This feature
will be time and energy saving for both the patients and
dentist. If you are a patient of the clinic, you will also
be given a temporary log-in id for you to fill-out a
patient record form. Once the patient has a patient record
in the system, he can now log-in with a desired username
and password.
Issuance of Bills.These features allow the clinic to
issue and record a bill to the patient and monitor the
clinics profit.
File Storage of Patient Records. Primary purpose of
this feature is to maintain dental records to deliver
quality patient care and treatment. Patient records are
electronically stored in a safe and sound manner. The
clinic need ample storage of these records in order to have
accurate information in practicing the dental profession.
Good record keeping is critical for a good dental practice
and it would be very helpful to the dentist.

Patient Records Management. The patient can update his
patient record. He can only update or modify the personal
information. The updates and modify of dental records can
only be done by the administrator or the dentist. The
patient can only view his patient records.

Chapter 4
IMPLEMENTATION PLAN
This chapter talks about the infrastructure of the
proposed online dental records information system of Lapea
Dental Clinic. It entails the network plan, System
requirements, budget and training plans.
Network Plan
The proposed online dental records system would use
the web host-web server setup. The required utilities and
resources to be used in developing the website of the
system would be uploaded or installed to a web host. All
other operations of the system pertaining to the database
would be handled by the server.

lifetime payment and upgrades and maintenance with the help

did not include the budget for hardware materials because
the clinic has two computers beforehand. The documentation
accommodates the system overview and manual.

Chapter 5
RECOMMENDATIONS
In this chapter the researchers includes
recommendation on the study which will allow the system to
be more productive and functional and improve its current
phase as well.
The following are the recommendation that may be
considered to improve the implementation of the proposed
system.
1. The system developer shall design a graphical
interface which will emphasize the intuitiveness of the
users in using the system, and also, include notifications
and guidelines to which will be beneficial in the
implementation of the Web-Based Dental System for Lapea
Dental Clinic.
2. The database of the system should be monitored and
maintained regularly to eliminate anomalies at the same
time provide an up to date events and other announcement as
well.
3. The study must be improved to include some aspects
which will contribute in finding other important features
that will be useful in terms of accommodating patients and
managing documents of Lapea Dental Clinic.

4. It is recommended that adding more feature will result
to the growth of the system which will allow it to be more
interesting, useful and entertaining to the users of it.
a. The system should consider recommendation of other
dentist if the initial dentist requested for consultation
is not available.
b. When adding new accounts for patient, it should not
be created outside the administrator account.
c. Use of calendar is strongly recommended for easier
input of dates.
