scholarship decisions
STEP 1: Save this file to your desktop and print a copy for your records.
This packet contains this checklist and your submitted application.
STEP 2: Pay the $36 application fee.
If you have not yet paid your application fee, log in at
www.msuadmissions.org/apply to pay securely online. If you prefer
to pay by mail, please submit your $36 check, made payable to
Montana State University, to the address above.

STEP 3: Make sure Montana State University receives your test scores.
Request your test scores from the appropriate testing service today. For your
ACT scores, go to http://www.actstudent.org/scores/send/ our code is 2420.
For your SAT scores, go to http://sat.collegeboard.org/scores/send-sat-scores
our code is 4488.
STEP 4: Ask your guidance counselor to submit your final transcript.
Wed like a record of your final senior-year grades after graduation.
Please ask your guidance counselor to send your official transcript and
graduation date directly to the Office of Admissions.

misrepresentation. I understand that if it is later found otherwise, it is sufficient cause for rejection or dismissal. If my application
for admission is approved, I agree to abide by the present and future rules and regulations, both academic and nonacademic,
and the scholastic standards of Montana State University, its colleges, schools, departments and institutes, including, but not
limited to, those rules, regulations and standards stated in the undergraduate/graduate catalog. I further acknowledge that if I fail
to adhere to these regulations or meet these requirements, my registration may be canceled.
If I enroll at Montana State University, I agree to pay all tuition, fees, fines and debts to the university that I may incur. I
understand that MSU will take action against me to collect any unpaid debts, including withholding of registration and transcripts
and assignment of the debt for collection, and I will be responsible to pay any costs incurred to collect the debt.
If I fail to pay any tuition or fees when due, I understand the university will treat any unpaid amount as an educational loan
extended to finance my education.
