Complete this report for suspects described or identified. Complete this
report if a suspect vehicle has been described or identified.
HEADER
Leave RD # blank.
Enter the crime from the original Crime Report.
Enter the incident number from the original Crime Report.
Enter the name of the primary victim from the original Crime Report.
Enter last name first, then first name and middle name or initial.
SUSPECT
Enter the suspect number.
Enter the name of the suspect if known. Enter last name first, then first
name and middle name or initial.
Enter the suspect's relationship to the victim (i.e., husband, wife, etc).
Indicate whether or not the suspect is in custody for this offense. If yes,
enter the citation number if appropriate.
Enter the suspect's sex, race, date of birth or age, height, weight, hair
color, eye color, driver's license number and PFN, if known.
Enter the suspect's home address including building, room or apartment
number, and zip code.
Enter the name of the city if outside Oakland. If Oakland address, darken
Oakland. For military personnel, include unit ship or station.
Enter the suspect's home telephone number, including the area code if
other than 510. For a business leave blank.
Enter the suspect's business or school address including zip code. Enter
the name of the city if outside Oakland. If Oakland address, darken
Oakland.

Enter the suspect's occupation or job title.

Enter the phone number of the suspect's place of employment.
ADMONISHMENT
Darken all boxes that apply. Indicate whether or not the suspect was
admonished. Indicate whether the suspect refused admonishment.
Indicate whether or not a statement was given. Indicate the name of the
officer administering the admonishment and taking the statement. Enter
the date and time the statement was begun.
Indicate if the suspect was on probation at the time. Indicate the county
and probation officer. Indicate if the suspect was on parole at the time.
Indicate the parole agent's name. Indicate if the suspect was a parolee
at large.
Enter the name of the person providing the description of the suspect
Enter the exact words used by the suspect when committing the
offense.
Describe any scars, marks, tattoos, clothing, etc.

When the suspect's vehicle was involved in the incident, indicate the
disposition of the vehicle (i.e., secured at scene, released to owner,
etc.). If none of the above dispositions are applicable, and the vehicle is
simply being described, leave Vehicle Was box blank. If towed, enter
the tow number. Include a description of any damage and/or unique
features. Include any other useful identifying features.
List the registered owner of the vehicle, if known, and the address of the
registered owner. If Oakland address, darken the Oakland box. Include
the zip code. Enter the home telephone number of the registered owner.
Fully describe the vehicle including the license number and the state.
Indicate the color and condition of both the exterior and the interior.
