MEDICINES

AGE: 28 years old

The client will take clindamycin 300mg per


capsule, one capsule three times a day by
mouth for seven days. Take this medication
at 8am, 1pm and 6pm with meals.

The client will take aceclofenac 100mg per


tablet, one tablet two times a day by mouth
for seven days. Take this medication at 8am
and 6pm with meals.
The client must not have any strenuous
activity like heavy weight lifting for at least 1
week to prevent wound incision problem.
May perform brisk walking as an exercise
everyday.

comfortable and stress free environment to
promote relaxation.
The client must strictly comply with the
course of taking the medications.

Caring for the T-tube drain

Dont sleep on the same side as the tube.


(Huwag matulog sa side kung nasaan ang
drain)
Secure the tube and bag inside your
clothing. This will prevent the tube from
being pulled out. (I-secure ang bag na may
drain sa loob ng damit upang hindi mahugot
at matanggal)
Tape plastic wrap over the bandage and
tube site when you shower (Kapag maliligo
ay ingatan mabuti ang bag para hindi
matanggal)

Call your doctor or seek consultation if:

Pain that becomes worse, especially in the


right upper portion of your abdomen
Nausea or vomiting
Fever above 101.5F (38.5C) or chills
Swelling or fluid around the tube
New redness or warmth around the incision
or new fluid draining from the incision
An incision that does not heal after 3 to 5
days
Stitches that become infected or loose
A foul smell from the incision site
Drainage that changes color from light pink
to dark red
A tube that falls out

Emptying the bag

Remove the closure at the bottom of the


bag. (Buksan ang opening ng bag sa ilalim)

Drain the fluid into a measuring cup. (I-drain


ang t-tube sa isang lalagyan na may
sukatan, upang masukat at maobsebahan
ang drain na lumalabas)

Record the amount of fluid each time you


empty the bag. You will share this
information with your doctor on your next
visit. (Sukatin at i-record ang nadrain para sa
susunod na check-up)

Wound dressing

The client must perform wound dressing


aseptically.
The client will only use sterile gauze and
antiseptic solution for cleaning the wound.

Remove the soiled dressing. Roll or lift an

edge of the dressing, then gently remove it


while supporting the surrounding skin.
(Tanggalin ang madumi o may dugong gasa
na nasa sugat habang sinusuportahan ang
balat na pinagdikitan)

Inspect the dressing and wound. Note the


color, amount, and odor of drainage and
necrotic debris. (Tingnan mabuti ang
tinanggal na gauze kung ano ang itsura o
may kakaibang amoy)

Clean the wound. Moisten gauze pads either


by dipping the pads in wound cleaning
solution and wringing out excess or by using
a spray bottle to apply solution to the gauze.
Move from the least contaminated area to
the most contaminated area and use a clean
gauze pad for each wipe. To clean a linearshaped wound, such as an incision, gently
wipe from top to bottom in one motion,
starting directly over the wound and moving
outward. (Gamitin ang gasang malins at
lagyan ng antiseptic o gamot na pangsugat
at ipahid sa sugat ng isang direksyon. Mula
sa
pinakamalinis

sa

Dry the wound. Using the same procedure


as for cleaning a wound, dry the wound
using dry gauze pads. (Tuyuin ang sugat
gamit ang malinis na gasa ipahid nang isang
direksyon katulad ng paglilinis)

Reassess the condition of the skin and


wound. Note the character of the clean
wound bed and surrounding skin. (Tingnan
din mabuti ang sugat kung ito ba ay
naghihilom o hindi)

Pack or dress the wound as ordered (Gamit


ang isang malinis na gasa ay takpan ang
sugat ayon sa pagkakatakip nito)

The client must perform proper hand


washing before and after cleaning the
wound.
The client will have a follow-up check up on
November 3, 2014 in Premiere Medical
Center Clinic between 10 to 11 in the
morning
The client may eat Vitamin C-rich fruits and
vegetables to promote wound healing and
boost body immunity.
The client may increase intake of lean meat,
which is high in protein, as this will help in
damaged tissue repair.
The client may continue routines that will
promote participation in her religion.
