INSPECTION NOTES
I attended the Meadow Creek Forest Products mill site with SRO Nick Morris to review progress of safe guarding and assist the
employer with development of safe guarding prior to mill start up.
Not limited to the following was reviewed and discussed regarding safe guarding while onsite:
- Old twin band saw operators booth
The safe guard lockable gate needs to be moved so the operator has a means of exiting in the event of
and emergency. The operator booth access door cannot be behind a locked door.
- Edger operator booth
Similar to the Old twin band saw operators booth the safe guard gate cannot be located such that the
operator is locked in and does not have a clear emergency escape path.
- Horizontal operator booth
Discussed ensuring that the local equipment safe guards are in place and having administrative controls
for accessing the booth area. Example: closed door with signs and operator authorized access.
- Stairs from saw filer room at the west end of the mill
Discussed modification to location of gates and fencing to allow for emergency egress via the west stairs.
- #2 Belt sorting station
Safe guarding is required on the back of the takeup.
- Sawmill chipper area
We discussed a number of safe guarding deficiencies in this area such as:
- Chip blower drive
- Chip conveyor drive
- Chip conveyor tail spool
- Chipper in feed vibrating conveyor drive
We advised the employer representative to ensure that gates and fencing used as safe guards do not trap workers in the event of
and emergency and to ensure workers have two ways of exiting their work area in the event of and emergency.
We also discussed plans for cleanup and how they fit into the gate and fenced area safe guards.

Fall protection saw filer room service hatches


We also discussed the plans proposed for fall protection in the saw filer area for when the floor service hatches are open.
I advised the employer representative that the plan or design must achieve the objective of ensuring the fall hazard is controlled
such that the risk of falling through the opening is eliminated.

Combustible Dust
I discussed that once the mill is in operation it will become a combustible dust inspection work site and to expect combustible dust
focused inspections. I advised they will need to develop a combustible dust management program.

INSPECTION NOTES
No further enforcement action will be taken at this time as a result of any orders issued in this Inspection Report. Further
enforcement action may be considered if the orders relate to an incident under investigation by WorkSafeBC or other
circumstances warrant.
I conducted a worksite inspection at the sawmill with review and discussion not limited to the following:
Worker Personal Protective Equipment (PPE)
A worker was observed not wearing high vis vest and hard hat while piling lumber in the yard.
First Aid
This high hazard workplace located more than 20 minutes surface travel time to a hospital with between 11 and 30 workers (14) is
required to have a OFA Level 3 attendant, a Dressing station and and ETV and equipment. See Table 5 of Part 3 of the OH&S
Regulation.
At the time of inspection I reviewed for:
- an OFA Level 3 attendant on site (An OFA Level 3 attendant was available on site)
- a Dressing station with equipment and cleanliness
- an ETV on site equipped and ready to transport injured workers
- First aid procedures as required
Safe Guarding deficiencies
At the time of inspection I observed safe guarding deficiencies
- Chip blower drive was not guarded. Guarding of the equipment in this area was discussed with the employer during the last site

- Center Canter Chain Drive
The metal guard for the drive belt and pulleys on the Center Canter Chain was not reinstalled after removal by a employee. At the
time of inspection this was not running (lunch break). The employer requested the millwright to install the guard.
- Gate to Out feed area of Horizontal
The employer has installed fence and locked gate system combined with lockout requirements in some areas of the mill as safe
guards. A lock was not installed on the gate from the cat walk to the out feed area of the horizontal saw.
Angle Grinder Guard
A Milwaukee 6 Inch angle grinder was being used with a zip disk to cut steel. There was no guard on the angle grinder. The
workers were advised to stop use of the grinder until the guard is installed. The RPM of the zip disk was reviewed and compared
to the RPM of the grinder. The disk had a higher RPM rating (8650) than the grinder (6000).
Combustible Dust
I discussed again that the mill is a combustible dust inspection work site and to expect combustible dust
focused inspections. I advised to develop a combustible dust management program.

This high hazard workplace located more than 20 minutes surface travel time to a hospital with between 11 and 30 workers (14)
is required to have a OFA Level 3 attendant, a Dressing station and an ETV and ETV equipment. See Table 5 of Part 3 of the
OH&S Regulation.
At the time of inspection :
- First aid attendant - an OFA Level 3 attendant was on site with a first aid jump kit and oxygen.
- Dressing station - cleanup had been started of the dressing station but the station was not equipped with the required first aid
supplies and equipment
- ETV - on site was a van designated as the ETV. The ETV was not equipped and ready for use. A pile of equipment was stored
in the rear area of the van, the rear area was dirty and the van was not licensed for use on the highway.
This employer has not provided for adequate equipment, supplies, facilities, first aid attendants and services to promptly render
first aid to injured workers.
This is in contravention of the Occupational Health and Safety Regulation Section 3.16(1)(a).
The employer must provide for each workplace such equipment, supplies, facilities, first aid attendants and services as are
adequate and appropriate for
(a) promptly rendering first aid to workers if they suffer an injury at work, and
(b) transporting injured workers to medical treatment.
Measures to Ensure Compliance:
The employer is required, without undue delay, to outfit the dressing station with the required first aid equipment, and equip the
ETV with required equipment and make it ready for use as an ETV.
Pursuant to section 194 (1) of the Workers Compensation Act, the employer must prepare a Notice of Compliance report. In
accordance with section 194 (2), this report must detail what has been done to comply with the order, and where compliance has
not been achieved by the time the report has been submitted, include a plan of what will be done to comply and when
compliance will be achieved. Please submit the report no later than September 9, 2014.

The employer has not developed up-to-date written procedures for providing first aid at the worksite.
This is in contravention of the Occupational Health and Safety Regulation Section 3.17(1).
The employer must keep up-to-date written procedures for providing first aid at the worksite including
(a) the equipment, supplies, facilities, first aid attendants and services available,
(b) the location of, and how to call for, first aid,
(c) how the first aid attendant is to respond to a call for first aid,
(d) the authority of the first aid attendant over the treatment of injured workers and the responsibility of the employer to report
injuries to the board,
(e) who is to call for transportation for the injured worker, and the method of transportation and calling, and
(f) prearranged routes in and out of the workplace and to medical treatment.
Measures to Ensure Compliance:
The employer is required to develop and implement the required first aid procedures for this workplace.
Pursuant to section 194 (1) of the Workers Compensation Act, the employer must prepare a Notice of Compliance report. In
accordance with section 194 (2), this report must detail what has been done to comply with the order, and where compliance has
not been achieved by the time the report has been submitted, include a plan of what will be done to comply and when
compliance will be achieved. Please submit the report no later than September 12, 2014 .

I observed the following safe guarding deficiencies:


- Chip blower drive was not safe guarded. The employer immediately requested a worker to install a safe guard.
- Center Canter Chain Drive
The metal guard for the drive belt and pulleys on the Center Canter Chain was not reinstalled after removal. At the time of
inspection this was not running (lunch break). The employer requested the millwright to install the guard immediately.
- Gate to Out feed area of Horizontal
The employer has installed fencing and a locked gate system combined with lockout requirements in some areas of the mill as
safe guards. A lock was not installed on the gate from the cat walk to the out feed area of the horizontal saw.

The employer has not ensured that machinery and equipment is fitted with adequate safeguards which protect a worker from
contact with hazardous power transmission parts, ensure that a worker cannot access a hazardous point of operation, and/or
safely contain any material ejected by the work process which could be hazardous to a worker.
This is in contravention of the Occupational Health and Safety Regulation Section 12.2.
Unless elsewhere provided for in this Occupational Health and Safety Regulation, the employer must ensure that machinery and
equipment is fitted with adequate safeguards which
(a) protect a worker from contact with hazardous power transmission parts,
(b) ensure that a worker cannot access a hazardous point of operation, and
(c) safely contain any material ejected by the work process which could be hazardous to a worker.
Measures to Ensure Compliance:
The employer is required to ensure machinery and equipment is adequately safe guarded either by the chosen system of locked
gates with fences which restrict area access and lockout procedures or by ensuring safe guards are installed on hazardous
power transmissions and hazardous points of operation.

This is in contravention of the Occupational Health and Safety Regulation Section 3.17(1).
The employer must keep up-to-date written procedures for providing first aid at the worksite including
(a) the equipment, supplies, facilities, first aid attendants and services available,
(b) the location of, and how to call for, first aid,
(c) how the first aid attendant is to respond to a call for first aid,
(d) the authority of the first aid attendant over the treatment of injured workers and the responsibility of the employer to report
injuries to the board,
(e) who is to call for transportation for the injured worker, and the method of transportation and calling, and
(f) prearranged routes in and out of the workplace and to medical treatment.

Oct 20, 2014 - The employer representative provided a Notice of Compliance report which states:
- Chip blower drive is guarded, center canter chain drive is guarded, gate to out feed area horizontal is locked
- working on fitting adequate safeguards throughout the mill.
I observed the following safe guarding deficiencies:
- Chip blower drive was not safe guarded. The employer immediately requested a worker to install a safe guard.
- Center Canter Chain Drive
The metal guard for the drive belt and pulleys on the Center Canter Chain was not reinstalled after removal. At the time of
inspection this was not running (lunch break). The employer requested the millwright to install the guard immediately.
- Gate to Out feed area of Horizontal
The employer has installed fencing and a locked gate system combined with lockout requirements in some areas of the mill as
safe guards. A lock was not installed on the gate from the cat walk to the out feed area of the horizontal saw.

The employer has not ensured that machinery and equipment is fitted with adequate safeguards which protect a worker from
contact with hazardous power transmission parts, ensure that a worker cannot access a hazardous point of operation, and/or
safely contain any material ejected by the work process which could be hazardous to a worker.
This is in contravention of the Occupational Health and Safety Regulation Section 12.2.
Unless elsewhere provided for in this Occupational Health and Safety Regulation, the employer must ensure that machinery and
equipment is fitted with adequate safeguards which
(a) protect a worker from contact with hazardous power transmission parts,
(b) ensure that a worker cannot access a hazardous point of operation, and
(c) safely contain any material ejected by the work process which could be hazardous to a worker.

INSPECTION NOTES
No further enforcement action will be taken at this time as a result of any orders issued in this Inspection Report. Further
enforcement action may be considered if the orders relate to an incident under investigation by WorkSafeBC or other
circumstances warrant.
I attended the mill site and conducted an inspection with review and discussion not limited to the following:
Site drinking water
The employer advised he would have the water tested to ensure it is potable.
Washrooms
Washrooms are provided for workers.

Lockout
I discussed with the employer the requirement to have written specific lockout procedures for every lockout of machinery and
equipment.
The employer provided copies of a "Generic Lockout Procedure" "Emergency Lock Removal Procedures"
The Horizontal operator was observed clearing a jam up in the out feed area of the horizontal line. The operator went to the
electric panel and opened a breaker, but did not install a lock, prior to going under the conveyor. I discussed with the worker and
employer the lockout requirements.

Safe Guarding
I discussed at length the safe guarding requirements. The area gate and fencing is not functioning to provide safe guarding for
workers. The employer advised they are going to guard equipment and move away from area safe guarding. I advised that in the
interim, while equipment safeguarding is being installed, they must follow the area safe guarding combined with safe work
procedures for authorized workers who enter a safe guard area.

The out feed chain of the horizontal saw was shut down for clearing of a jam up. The operator opened a breaker but did not lock
out with a personal lock prior to performing work on the jam up.
This is in contravention of the Occupational Health and Safety Regulation Section 10.3(1)(c).
If machinery or equipment is shut down for maintenance, no work may be done until the energy isolating devices have been
locked out as required by this part.
Measures to Ensure Compliance:
The employer must ensure the all workers required to lockout are following lockout procedures which includes securing isolation
devices with the use of locks.
Pursuant to section 194 (1) of the Workers Compensation Act, the employer must prepare a Notice of Compliance report. In
accordance with section 194 (2), this report must detail what has been done to comply with the order, and where compliance has
not been achieved by the time the report has been submitted, include a plan of what will be done to comply and when
compliance will be achieved. Please submit the report no later than October 29, 2014.
