JOINT AFFIDAVIT OF
TWO DISINTERESTED PERSONS
We, LEONILO M. GO and MICHAEL S. SY, both Filipinos, of legal
ages, married and residents of St. Francis Village, Malhacan, Meycauayan,
Philippines, having been duly sworn in accordance with law, hereby depose
and state:
1. That we both personally know the person of GLORIA R.
LABANDERA since her childhood because formerly, we had lived in
the same locality for a long time, and her family has been a close
family friend;
2. That we also know and we hereby declare that the said GLORIA R.
LABANDERA was born on May 24, 1980 at Meycauayan, Bulacan,
Philippines to PERLA R. LABANDERA and ARIEL LABANDERA;
3. That we are not related in any way, either by consanguinity or
affinity, to the said GLORIA R. LABANDERA;
4. As such, we execute this Joint Affidavit to attest to the truth of the
foregoing facts and for any other legal purpose that this Affidavit
may serve.

I, JUAN DELA CRUZ, of legal age, married, Filipino and a resident
of Zone 1, Concepcion Grande. Naga City, Philippines having been duly
sworn

accordance

law,
hereby
depose
and
state:
1. That on May 2, 2009, I was issued a Philippine Passport with
passport number EB123456 in which my true surname DELA CRUZ was
stated;
2. That before the expiration of the aforementioned passport on
May 2, 2014, I had applied with Philippine Embassy in Legaspi City for the
issuance of a new passport and on June 10, 2014, I was issued a new
Philippine
passport
with
passport
no.EB123456;
erroneously

3. However, in the said new passport my surname had been

misspelled
as
DELA
CRUX;

4. That I execute this Affidavit of Discrepancy to attest to the

truth of the foregoing facts and to explain the discrepancy in my Philippine
Passport
with
regards
my
surname.
