1. Complete Support Program intake assessments of individual or


family/caregiver
2. Develop an Action Plan to deliver support and advocacy in
groups and one to one settings
3. Provide Supportive Counseling to individuals and families
affected by acquired brain injury
4. Work collaboratively within an interdisciplinary client services
team
5. Review intake information to develop support objectives and
goals to be addressed in counselling sessions
6. Assess the appropriateness of referrals to the support program
7. Initiate referrals to community service providers for clients that
have been deemed inappropriate for the Support Program
8. Facilitate Life Skills classes that complement those at the
Glenrose Rehabilitation Hospital
9. Advocate for clients and caregivers as necessary
10. Liaise with families in transition from hospital to community to
provide support
11. Develop and maintain working links with other relevant agencies
in the community

1. Disseminate knowledge regarding adjustment counseling, brain


injury and related topics to community partners
2. Work collaboratively with external service providers in the
support of client care plans

Public Education

3. Work with the Public Education Committee in the delivery of


Brain Basics Education Sessions to various organizations,
professionals and caregivers
4. Provide education to Supports for Community Living Services
staff to enhance their client support capacities when requested

Reporting

5. Add client identified counseling needs/issues and case notes to


the File-maker database
6. Complete client progress ratings and quarterly reports for
submission to the Program Lead

7. Be familiar with and adhere to proper administrative procedures


8. Participate as a team member
9. Adhere to high standard of ethical practice
10. Have a working knowledge of Brain Care Centres Policy and
Procedures
11. Be comfortable with public speaking

Position Detail

Full time/temporary, 37.5 hrs./week, 6 12 month contract position,


paid staff parking and benefits after 6 months. Start date could
be as early as April 9, 2015 but no later than April 30th. This is a
maternity leave coverage position.

Preferred Competencies

Masters Degree in Counseling Psychology. Consideration will also


be given to candidates with Bachelor Degrees in Psychology or
Social Work who have a strong background in counseling and group
facilitation.
