The short-form 36 health survey is a patient-reported survey of patient
health. It includes one multi-item scale that assesses eight health concepts: (1)
physical functioning; (2) role limitations because of physical health problems; (3)
bodily pain; (4) social functioning; (5) general mental health (psychological
distress and psychological wellbeing); (6) role limitations because of emotional
problems; (7) vitality (energy/fatigue); and (8) general health perceptions. Te
survey was constructed for self administration, and for administration by an
experienced interviewer in person. In a study about validation, the response rate
for the SF-36 questionnaire was high (83%) and the rate of completion for each
dimension was over 95%. Considerable evidence was found for the reliability of
the SF-36.
