Instructions for Filling the Application Form

1.

All the entries in the form must be made in block letters or typed and no column should be left blank.

2.

There will be no bank service charges for online submission of processing fee through any branch of
Allied Bank Limited (ABL).

3.

Application processing fee must be deposited in the online Account No. 0010002084620041 (Title of
Account: PIEAS-APF), Allied Bank Limited (ABL), Cabinet Division Branch (Code No. 0793),
Islamabad.

Preferably apply online; application forms are available at PIEAS website. However, receipt of bank
deposit along with a passport size photograph must be sent to Registrar by post before the deadline.

Application forms may be downloaded from PIEAS website. Duly filled Form, a Photograph and Bank
Receipt must be sent to Registrar by Post before the deadline.
