Dear Sir/Madam,
Greetings in the name of St. La Salle!
Having the power, authority and duty to examine and audit all financial accounts of
every USG unit, we would like to request for the following documents for the 2nd term of
the current academic year:
The copies of documents required to be submitted shall include but not limited to
the following:
a. Financial Record Book
Hard copy
Soft copy: dlsu.officialcoa@gmail.com
b. Income Statement (Operational and Depository Fund)
Hard copy

c. Statement of Government Units Responsibility (signed)
There could, however, still be additional requirements which at the moment cannot be
anticipated in which case a separate request for such is to be expected.
Please hand it to us the earliest time possible. The deadline for your compliance will be
on January 13, 2015 but the sooner, the better. Kindly contact name of assigned chief
auditor at cellphone number and email address. Failure to comply with the requirement
will compel us to send your office a letter of demand and if there has been no response
to the Letter of Demand within three days after the date stated in the aforesaid letter, a

Notice of Audit Disengagement will be sent and we will issue a disclaimer of opinion,
which is tantamount to undesirable.
Note: Copies of documents submitted for review and evaluation should be duly certified
as true/correct copies of the originals. Also, attached with this letter is a checklist of the
requirements which is to be submitted together with the requested documents.
Please send the soft copy via email to dlsu.officialcoa@gmail.com with the subject:
AUDIT DOCUMENTS, name of office, Term (e.g. AUDIT DOCUMENTS, OTREAS,
TERM 2) and furnish a hard copy to be sealed in a long brown envelope bearing the
signature of your units head and addressed to COA. Attach the checklist on the
envelope
