AAD 325: Practicum II Service to the Field
This form should be completed in collaboration with your site supervisor. When all parties have agreed on
the content and all revisions of this document, prepare a printed final draft for signatures and distribution to
all parties. It is your responsibility to ensure that you, your site supervisor, and the professor all have
copies of this agreement.

PART III: THE POSITION

A. JOB DESCRIPTION: Please provide a complete job description. This description should include
detailed information about your internship role and responsibilities including duties, projects, and
information that defines what you will be doing throughout your time with the organization.
B. LEARNING OBJECTIVES: Please provide a short statement regarding the learning objectives of
your internship experience and how you intend to achieve those objectives. What do you intend to
learn through this experience? What abilities or knowledge do you intend to develop or enhance?
How will you do it? Please be specific and use concrete, measurable terms for the learning
objectives. Include projects, research, report writing, and informal interviews you plan to conduct
while working.
C. HOURS: You are strongly encouraged to establish your work schedule for the entire semester up
front. Required hours: 47 working hours this does not include commute time. If you will be
working regular weekly hours throughout your internship, please provide them below and have
your site supervisor initial under the schedule:

If your hours will vary (due to your duties) then please provide a brief statement explaining why
your internship position will require you to work variable hours on a separate sheet of paper (may
be included on the same document as items A and B). Your site supervisor must initial this
statement.
During your time at the organization, you are required to keep a log of the hours that you work. An
example of this log is available on the Canvas Site. You must have the log signed by your site
supervisor at the end of the semester. You will submit your log with the rest of your final materials.
D. SUPERVISION: Site supervisors are required to offer written feedback at two points during the
semester: a midterm evaluation and a final evaluation. In addition to the written evaluation, a phone
discussion with the professor is required. These meetings will be scheduled via email once this
form is submitted. Site supervisors must initial this section to acknowledge their obligation to
submit feedback in a timely manner, as late responses may impact students grades.

E. EVALUATION: During the course of your internship your evaluation will be determined by the
following items:
1. A mid-term evaluation (to be conducted by your site supervisor)
2. A mid-term meeting with your professor
3. A final evaluation (to be conducted by your site supervisor)

4. A final meeting with your faculty sponsor


5. Documentation of the hours you worked (signed by your site supervisor)
6. A final paper (5-7 pages) that consists of a description of your duties, a discussion of
your learning objectives and whether you achieved them, a discussion of the things
you enjoyed about your experience and the things you would change, a discussion of
how the experience has impacted your future career goals, and a discussion of the
next steps you will take with regard to your professional development. It is expected
that information from class sessions will be incorporated into the paper. For Summer I
the final paper is due by Friday, June 3.
7. An assessment of your experience
F. SPECIAL NOTE: You are strongly urged to keep a copy of all the materials you create or
participate in creating during your internship for your own portfolio and to show your faculty
sponsor during your mid-term and final meetings. Be sure to obtain the permission of your site
supervisor before copying/sharing any materials from your internship.
If, at any point in your internship, you act in any way that would result in a reasonable
expectation of termination you will receive a F for the semester.
PART IV: AGREEMENT
This agreement may be terminated or amended by the student, professor, or site supervisor upon written
notice, which is received and agreed to by the two other parties. This is a working agreement and does not
constitute a contractual obligation for any of the parties concerned.
