IcertifythattheinformationonthisApplicationFormiscomplete,trueandaccurate.
IunderstandthatthepersonaldataonthisApplicationFormiscollectedforthepurposesoftheIELTS
test,andIconsentforthisdatatobedisclosedto,processedandstoredbytheIELTSTestPartnersfor
thepurposeofsuchadministration.Ifurtherconsentforthisdata,including,withoutlimitation,test
performanceorscoredataorphotographstakenbytheIELTStestcentreandmytestresulttobe
disclosedbytheIELTSTestPartnerstothoseRecognisingOrganisationstowhichIapplyandto
governments(includingvisaprocessingauthorities)forthepurposeofallowingtheseorganisationsto
verifymytestresultortocarryoutenquiriesinrelationtosuspectedmalpractice.IftheIELTSTest
PartnersdiscoverthatafalseoralteredTestReportFormhasbeenprovidedtoanyofthese
RecognisingOrganisationsorgovernmentauthoritiesIfurtherconsentthattheIELTSTestPartners
mayinformthesameandprovidethemwithmypersonaldataandanyrelevantdetailsrelatingtothe
workIproduceaspartofmytesttaking,including,withoutlimitation,testperformanceorscoredataor
photographstakenbytheIELTStestcentre.
Iunderstandthatmypersonaldatamaybeprocessedinananonymousformforstatisticaland
researchpurposesforthedevelopmentofCambridgeEnglishLanguageAssessmentexaminations.

bereleased.IfIhavenothadmyphotographtakenbythetestcentrenoresultwillbeissued.
IacknowledgethatIhavereadtheIELTSNoticetoCandidatescontainedonpageiofthisdocument
andagreetoabidebytherulesandregulationscontainedtherein.
IunderstandtheremaybelocaltermsandconditionsImustcomplywithandthatthetestcentrewill

Iunderstandthatmyresultmaynotbeissued13daysafterthetestifanyoftheIELTSTestPartners
deemitnecessarytoreviewanymatterassociatedwithmytestortheadministrationofmytest,
includingmakingenquiriesastowhetheranyrulesorregulationshavebeenbreached,asoutlinedin
theIELTSNoticetoCandidates.IunderstandthatinexceptionalcircumstancesImayberequiredto
retakeoneormoreIELTScomponents.

IunderstandthatifIamsuspectedofengaginginanyformofmalpractice,ordoanythingthatmight
damagetheintegrityandsecurityofIELTS,Imaynotreceiveatestresult,mytestfeewillnotbe
refundedandImaybeprohibitedfromtakingtheIELTStestinthefuture.Despiteandwithoutlimiting
anyofthetermsofthisDeclaration,Iunderstandthatdetailsofanymalpractice(includingevidenceof
suspectedmalpractice)thathasbeenestablished,issuspected,orisbeingformallyinvestigatedmay
beprovidedtoRecognisingOrganisations,includingvisaprocessingauthoritiesandappropriate
regulatoryauthorities,orotherwisedisclosedinaccordancewiththelaw,whererequiredfor
verificationpurposesorotherpurposestoprotecttheIELTStestanditsstakeholdersagainstanyform
ofmalpractice.IfurtherunderstandthatsuspectedmalpracticewillbereportedcentrallytotheIELTS
TestPartnersandtoanyrelevanttestcentrebythecentrewherethesuspectedmalpracticeoccurred.
IunderstandthatifanyotherpersonattemptstotaketheIELTStestinmyplace(i.e.inplaceofthe
personwhosedetailsappearonthisform),bothIandsuchpersonwillbeliabletoprosecution.
Detailsrelatingtothesituationmaybeprovidedtotherelevantauthorities,includingvisaprocessing
authoritiesandappropriateregulatoryauthorities.
IunderstandthattheworkIproduceaspartoftheIELTStestremainsthepropertyoftheIELTSTest
Partners.Undernocircumstanceswillitbereleasedtocandidatesortoinstitutionsororganisations,
exceptintheinvestigationofsuspectedmalpracticewherebytheworkIproduceaspartoftheIELTS
testmaybeprovidedtorelevantauthorities.
IunderstandthatmySpeakingtestwillberecorded.Ialsoagreethatanobservermayattendmy
Speakingtestaspartofthemonitoringprocess.
IunderstandthatIwillbechargedthefulltestfeeifIcancelmytestorrequestatransferwithinfive
weeksofthetestdate,unlessIprovideappropriatemedicalevidence,withinfivedaysofthetestdate,
tosupportthecancellationortransfer.
