Chile Earthquake

Symbols of Hope
The South American nation of Chile has
experienced wide-spread devastation
5k/Fun Run
from an 8.8 magnitude earthquake that
hit on February 27th, 2010. With a need
to rebuild many portions of their Saturday, April 24th, 2010
country, we have an opportunity to 10:00am
serve and lend a helping hand to those Nampa, Idaho
less fortunate.

All proceeds generated from this event

will go directly to those affected by
earthquake in Chile.

 PARTICIPANT WAIVER
This must be signed in order to participate. This
waiver also applies to all of your group participants.
This release and waiver is executed on this date, Saturday,
April 24th, 2010. Knowingly, and at our own risk, I and
members of my group, are participating in the Symbols of
Hope 5k/Fun Run. We do hereby waive release of any and
all claims against the Symbols of Hope organizers, Triple
Crown Estates HOA, Vineyard Pointe HOA, all event
sponsors and any volunteers, or officials of these
organizations from any claim of damage to property or
injury (including death) that I, or members of my group,
may incur as a result of our participation in the event. I
further hereby certify that I have full knowledge of the
risks involved in this event, and we are physically fit and
sufficiently trained to participate. If, however, as a result
of our participation in the Symbols of Hope 5k/Fun Run, I or
any member of my group require medical attention, I
hereby give consent to authorize medical personnel to
provide such medical care as deemed necessary to myself
or any member of my group.

Both events will begin at the center grassy park in
Triple Crown Estates subdivision in Nampa, ID
(entrance is near the corner of Greenhurst and Happy
Valley). Overflow parking will be at the LDS Stake
Center on 3904 E. Greenhurst Rd. Route details will
be available on the website.
