Description of work to be performed and services to be provided:
1 Furnish labor and materials to add insulated safety glass to one office door at the above mentioned address as follows:

2 Remove existing door, transport to M&J shop sometime in the am.

3 Cut opening in door to match existing door previously finished by others

4 Dado hardwood custom molding to act as glass stops and trim glass opening

5 Miter and attach custom molding/glass stops one side

6 Silicone seal and install glass envelope 3/4" safety glass

7 Miter, silicone and install second side of custom molding

8 Paint to match previously finished door

9 Install finished door pm. Same day.

Any changes to this contract must be in written form.

We propose hereby to furnish material and labor, complete in accordance with the above specifications, for the sum of:
FOUR HUNDRED SEVENTY-FIVE DOLLARS (475.00)

Payments to be made as follow:

NET 30 DAYS

All material is guaranteed to be as specified. All work to be completed in a workmanship like manner according to standard practices. If the client requests deviations from
the above specifications that represent added costs beyond the time and materials quoted, a separate written addendum will be required before the alterations can be executed.
The changes requested by the client will become an additional charge over and above the originally quoted price. The client agrees that M&J General Contracting will not be
held liable for delays caused by labor strikes, accidents, adverse weather conditions, or other factors beyond the control of M&J General Contracting. The employees and sub-
contractors of M&J General Contracting are fully covered by Workman's Compensation Insurance.

Acceptance Of This Contract

The above prices, specifications, and conditions are satisfactory and are hereby accepted. Signatures below authorize M&J General Contracting to commence work on the
project. Payment for the project will be made as detailed above. Client acknowledges that they carry adequate levels of hazard insurance on the location/premises in which
the work will be performed.
