THE EFFECTIVENESS disposal of secretions by coughing and breathing
awareness, effective in tuberculosis patients in a general hospital center

ABSTRACT
The classic symptoms of active TB chronic infection is cough greenish sputum,
fever, sweats at night and weight loss. The phenomenon of the entry of germs of
tuberculosis will infect the lower respiratory tract and can lead to a productive
cough and blood. The decrease of cilia work function caused accumulation of
secret in the respiratory tract causing a nursing diagnosis of ineffective airway
clearance due to the accumulation of secretion. The survey conducted at the
General Hospital Center of H. Adam Malik obtained data, the number of patients
who suffered tuberculosis in 2011 as many as 847 patients, 936 patients in 2012,
and 132 patients in 2013 to 14th May 2013. The experimental research aims to
determine the extent of the effectiveness of effusion secretions by technique of
deep breathing and effective coughing in tuberculosis patients at the General
Hospital H. Adam Malik. Thirty patients met the criterion were taken as the
samples. The characteristics of respondents are age, sex and vital signs
measurements are made as a confounding variable. Bivariate analysis using a
dependent samples t-test showed a significant effect of exercise effective cough
and deep breath to the secretions effusion in the intervention group (p value =
0.000) whereas in the control group (p value 0.076). The research results indicate
there is effect of effective cough exercise and deep breathing technique in effusion
of secretions on TB patients with disorders of airway clearance. For nurses need
to conduct this exercise scheduled and integrated for all TB patients are treated in
hospital.

DALAM DAN BATUK EFEKTIF PADA PASIEN TB DI RUMAH SAKIT
UMUM PUSAT H. ADAM MALIK MEDAN

ABSTRAK
Gejala klasik infeksi TB aktif batuk kronis dengan dahak berwarna kehijauan,
demam, keringat malam, dan penurunan berat badan. Fenomena yang terjadi
masuknya kuman tuberkulosis maka akan menginfeksi saluran nafas bawah dan dapat
menimbulkan terjadinya batuk produktif dan darah. Terjadinya penurunkan fungsi kerja
silia mengakibatkan penumpukan sekret pada saluran pernafasan sehingga menimbulkan
diagnosa keperawatan bersihan jalan nafas tidak efektif berhubungan dengan
penumpukan sekret. Survei yang dilakukan pada Rumah Sakit Umum Pusat H.

Adam Malik didapatkan data, jumlah pasien yang menderita penyakit TB

sebanyak 847 pasien ditahun 2011, 936 pasien ditahun 2012, dan 132 pasien
ditahun 2013 sampai dengan tanggal 14 Mei 2013. Penelitian eksperimen ini
bertujuan untuk mengetahui sejauh mana efektifitas pengeluaran sekret dengan
tehnik napas dalam dan batuk efektif pada pasien TB di Rumah Sakit Umum
Pusat H. Adam Malik Medan. Tiga puluh pasien sesuai dengan kriteria diambil
sebagai sampel.. Karakteristik responden yaitu umur, jenis kelamin dan
pengukuran tanda-tanda vital yang dijadikan sebagai variable perancu. Analisa
bivariat dengan menggunakan dependent samples t-test menunjukkan pengaruh
yang signifikan latihan batuk efektif dan napas dalam terhadap pengeluaran sekret
pada kelompok intervensi (p value 0,000) sedangkan pada kelompok kontrol (p
value 0,076). Hasil penelitian ini menunjukkan ada pengaruh latihan batuk efektif
dan napas dalam terhadap pengeluaran sekret pada pasien TB dengan gangguan
bersihan jalan napas. Untuk perawat perlu melakukan latihan ini secara terjadwal
dan terpadu untuk semua pasien TB yang dirawat di ruang inap.
