I, Nallapati Gowramma @ Gowthami D/o Kotaiah, Hindu, aged about 19
years, resident of Busarapalli Village, Chimakurthi Mandal, Prakasam District.,
do hereby solemnly affirm and sincerely state as follows:I am the deponent herein and 3rd petitioner in the main O.P. The O.P. is
filed for compensation for the death of my Mother, who died in a motor
accident. Later the matter was settled before the Lok-Adalat for an amount of
Rs.1,80,000/- towards full and final settlement on 09-11-2003.

The 2nd

respondent deposited an amount of Rs.1,80,000/- into the Court. I am entitled

to a share of compensation an amount of Rs.50,000/-.

At the time of filing

O.P., I was minor and my father represented me as guardian. Now I attained

majority on 06-05-2011 and the Honble Court permitted me to withdraw my
entire share of compensation amount with interest of Rs.81,689/-. The above
said amount of Rs.81,689/- was lying in C.C.D. I am entitled to withdraw the
same and there are no attachments or stay on the said amount. I, under take that
if the Honble Court directs me to re-deposit the amount, I will deposit the
same. I am herewith filing my Bank Pass-Book.
Hence I pray that this Honble Court may be pleased to issue a cheque for
Rs.81,689/- (Rupees Eighty one thousand and six hundred eighty nine Only) in
favour of the Branch Manager, The Prakasam District Co-Operative Central
Bank Ltd., Ongole Branch, Prakasam District, and he may be directed him to
credit the same to the S.B.A/c No.4639, of the petitioner in the interests of
justice.

Solemnly affirmed and signed before me the contents of the affidavit is read over and
explained in Telugu to the deponent and admitted her to be correct on this the
day of
June. 2011.
