IMPORTANT INSTRUCTIONS FOR FILLING UP THE EXAMINATION FORM

1. This form will be rejected if the entries regarding full name, subjects and other items of information are not
clearly mentioned in the form and if the required Documents, which are also provided in his form are not
properly filled in and are not authenticated by the proper authorities.
2. Form submitted after the prescribed date shall not be accepted.
3. Whatever is indicated in the form shall be considered as final.
4. The exemptions once exercised by a candidate shall be considered as final.
5. All entries in the form should be made in BLOCK LETTERS.
6. Form should be thoroughly checked before it is forwarded to the Postgraduate Examination Board,
Shaheed Mohtarma Benazir Bhutto Medical University, Larkana. The entire responsibility for any eventual
mishap at Examination on account of any inaccuracy or omission in the form shall be on the candidate.
7. The authorities of Shaheed Mohtarma Benazir Bhutto Medical University shall not be responsible for
any eventual mistake, in the result of candidate, if the Form is not correctly filled in, and if there is any
omission of subject/ paper of the material fact.
8. The candidate is required to attach the following documents with this form (duty attested photocopies).
a. MBBS Degree.
b. Valid PMDC Registration Certificate.
c. One year house job certificate (at least six months in the subsequent specialty).
d. Three fresh passport size photographs.
e. Copy of C.N.I.C. (Computerized National Identity Card).
f.

Original copy of challan form of authorized bank acknowledging the prescribed examination fee
(non-refundable).

g. Any other additional degree / diploma certificate.

IMPORTANT INSTRUCTIONS FOR CANDIDATES

1. Candidates are requested to reach the venue half an hour before the beginning of examination and make sure
to enter the examinations hall 15 minutes before the start of examination.
2. Candidates are instructed to present the Admit Slip issued by the University and C.N.I.C.
(Computerized National Identity Card) in each paper.
3. Mobile phones, cameras, notes, books, printed material, writing pad and all communication devices are
strictly not allowed within the examination hall and submit your belongings 30 minutes before the start of
examination at the designated counter of the examination hall.
4. Candidates must bring his / her own Ink, Pen, Pencil, Rubbers and other stationary articles.
5. Before solving, please make sure that you have received the question paper of your own subject.
6. Smoking is prohibited in the Examination Hall.
7. Candidates will not be allowed to use of wash room in the first one and a half hour and the last 30 minutes of
the examination time. Use of wash room will be monitored by supporting staff.
8. Candidates shall not be permitted to leave their seats on any account. If any candidate requires anything,
he/she may draw the attention of the Invigilator by standing up.
9. Candidates shall not be allowed to appear in any Examination center except the center mentioned
in the admit slip issued by the University. Any change of center shall only be valid if duly notified by the
Controller of Examinations, Postgraduate Examination Board, SMBBMU, Larkana.
10. Any candidate found guilty of misconduct or use of unfair means during the examination are liable to be
debarred for any specified period.
