Inc., as of ___28/05/2015_____________(DATE) to act as a Forwarding Agent for export control and to transmit
any Electronic Export Information (EEI) to U.S. Census, which may be required by law or regulation in
connection with the exportation or transportation of any merchandise on behalf of said Authorizing Party. The
Authorizing Party certifies that necessary and proper documentation to accurately complete the EEI will be
provided to the said Forwarding Agent one (1) business day** prior to date of export. If the proper
documentation is not provided by the Authorizing Party within the required time frame, Crowley Logistics, Inc
will not be held responsible for U.S. Customs fines or penalties, Ocean Transportation charges, export delays
or other liabilities involved as a result of the EEI filing. The Authorizing Party further understands that civil
and criminal penalties may be imposed for making false or fraudulent statements or for the violation of any
United States laws or regulations on exportation and agrees to be bound by all statements of said agent
based upon information or documentation provided by the exporter to the said agent.

**The Responsible Party must ensure designated documents containing shipment information required to file the
EEI are complete, accurate and submitted to CLI within ONE business day prior to the vessel sailing before 3:00 pm
EST. Not following this instruction can result in delay of the export shipment. (FTSR Reference 30.12)

Items subjected to Export Administration Regulations (EAR) will need the EAR Number.
Export Control Classification Number (ECCN).
It is the primary responsibility of the USPPI or FPPI to submit information to CLI before 3:00 PM EST (1)
business day prior to vessel sail date before 3:00pm EST. For Saturday and Sunday Sailings cut-off will be
3:00PM on Friday prior to sailing.

A routed Export is when the Foreign Principal Party (FPPI) authorizes a U.S. Forwarding Agent to export
merchandise out of the U.S. on their behalf.

Location from which the merchandise actually starts its journey to the port of export or from which the export
was directed, based on FTSR 30.7(d)(3).
