recommendation for disciplinary action due to your violent behavior and
unprofessional attitude towards Terry Thomas. This kind of behavior is not tolerated
and is not accepted under the Code of Conduct of FAAW.

Based on my review of the information gathered on the fact finding meeting
conducted on 4 December 2015 and due to the seriousness of the situation, you are
asked to submit a letter on 7 December 2015 on why you should not be suspended
for three (3) working days without pay for her unbecoming and quarrelsome
behavior. This is in line with Section 45 of the Code of Conduct of FAAWs
employees.

It is expected that you will immediately address this situation. Should you
require assistance or have any concerns going forward, please do not hesitate to
