Distribution.
This procedure is distributed in accordance with the Document Distribution List.

1.4

Purpose.
The purpose of this procedure is to provide establish minimum standards requirements
for working on unprotected work areas above 1.8 meters in height. This will apply to all
location within the CV. Dira Utama Sejahtera sites and or operations. The minimum
standards for training of personnel to ensure 100% understanding and compliance of CV.
Dira Utama Sejahtera fall protection program.

1.5

Scope.
This procedure is directly applicable to CV. Dira Utama Sejahtera operations in Indonesia.

1.6

Term and Conditions.


Employees any personnel employed on the DUS project site whether working for the
Contractor or subcontractor or for DUS.
Leading Edge :
The edge of a floor, roof, or formwork for a floor or other walking/working surface (such
as the deck) which changes location as additional floor, roof, decking, or formwork
sections are placed, formed, or constructed. A leading edge is considered to be an
"unprotected side and edge" during periods when it is not actively and continuously
under construction.
Leading Edge :
The edge of a floor, roof, or formwork for a floor or otherwalking/working surface (such
as the deck) which changes location asadditional floor, roof, decking, or formwork
sections are placed, formed,or constructed. A leading edge is considered to be an
"unprotected sideand edge" during periods when it is not actively and continuously
underconstruction.
Lifeline :
A component consisting of a flexible line for connection to ananchorage at one end to
hang vertically (vertical lifeline), or forconnection to anchorages at both ends to stretch
horizontally(horizontal lifeline), and which serves as a means for connectingother
components of a personal fall arrest system to the anchorage.
Opening : A gap or void 75cm or more high and 45cm or more wide, in a wall orpartition,
through which employees can fall to a lower level.
Personal Fall Arrest System :
A system used to arrest an employee in a fall from a working level. Consists of an
anchorage, connectors, and full body safety harness, and may include a lanyard,
deceleration device, lifeline, or suitable combinations of these.
Positioning Device System :
A full body safety harness system rigged to allow an employee to be supported on an
elevated vertical surface, such as a wall, and work with both hands free while leaning.

Snap Hook :
A connector comprised of a hook-shaped member with a normally closed keeper, or
similar arrangement, which may be opened to permit the hook to receive an object and,
when released, automatically closes to retain the object. Snap hooks will be the locking
type with a selfclosing, self-locking keeper which remains closed and locked until
unlocked and pressed open for connection or disconnection.
Rope Garb : A deceleration device that travels on a lifeline and locks so as arrest the fall
of an employee.
Secondary Fall Protection System :
Utilization of fall arrest equipment as back up to primary fall prevention system or in the
absence of primary fall prevention.

Responsibility.
HSE Coordinator will be responsible to issue, update and maintain this Procedure.

1.8

Procedure.
A fall protection plan will be written specifically for each project when fall protection is
required.
a. Fall Protection Program.
Supervision will plan and implement the use of primary fall protection systems such as
scaffolds, aerial lifts, and approved personnel hoists whenever possible. These systems
will have complete guardrail systems free from floor holes and safe access will be
provided. Employees will use a full body safety harness with two shock absorbing
lanyards is required when working higher than 1.8 meters unprotected. Full body
safety harnesses will be required for steel erectors, pipe erection, scaffolding, erection
of cables trays, siding and roof workers and any other unprotected situation.
Employees will use approved safety harnesses with 2 shock absorbing lanyards
wheneverworking above 1.8 meters where exposure to falls exists, or the use is
required in combination with the primary fall protection system.
Employees traveling or working in elevated areas 1.8 meters or more above ground
level, platform, or adjacent area will have one of the lanyards attached and secured at
all times to an anchorage, structure, lifeline, or approved fall arrest system capable of
supporting 2.25 tons per person or designed and installed as a complete fall arrest
system with a safetyfactor of 2 for the anticipated shock load. A designated competent
person will visually inspect body safety harnesses and lanyardsbefore initial use and
monthly. Body safety harnesses and lanyards will be visually inspected by the user
before each use. The body safety harness and lanyard will be color coded with tape to
verify current monthly inspection. Lifeline systems will be inspected by a designated
competent person weekly, or more often if necessary. Any defective equipment will
be removed from service immediately. Any fall protection device that has been
subjected in service to shock loading will be removed from service immediately. The
Project Manager/Site Coordinator and Site Safety Officer will be notified immediately.
Fall protection devices will not be used for any purpose other than fall protection.

b. Fall Protection System Requirements.


Primary Fall Protection
Primary fall protection systems are the preferred choice for performing work, such as
standard guardrails, solid floor with complete standard guardrails, patented or
wooden scaffolding (including bracket type), aerial lifts (arm type, scissors lift )
approved personnel hoist, etc., will be used whenever possible to provide fall
protection. These systems will becomplete walking/working systems with standard
guardrails, free of floor holes, and have a safe means of access/egress. In most cases
primary fall protection system are sufficient and do not require additional methods.
Secondary fall protection system should only be utilized after all effort to employ
primary fall prevention systems has been exhausted or when being used in
conjunction with primary systems.
Guardrail Systems
Guardrails system is an integral part of many fall prevention and protection system. It
must be constructed with absolute certainty of integrity and structural soundness with
the minimum standards.
Top rail must be 1.1 meters high plus or minus 50mm above the walking/working
surface.
Midrails will be installed halfway between top edge of guardrail and toe board of
working surface. When used, screens and mesh will extend from the top rail to the
walking/working level along the entire opening between the top rail supports.
Intermediate vertical members, when used between posts, will not be more than 19
inches apart.
Guardrail systems will be capable of withstanding, without failure, a force of 90kg
downward or outward and not deflect to a height of less than 1metre above working
surface when applied downward.
Midrails, screens, and intermediate vertical members will be capable of withstanding
70kg in any downward or outward force.
Guardrail systems will be smooth to prevent any snagging of clothes and punctures or
lacerations of employees. Toe boards are to be fitted to all guardrails system, the gap
between mid rail top rails /toe boards should not exceed 450mm Wire rope, 12 mm
minimum, may be used as a top rail and midrails. All wire rope used as guardrail must
be flagged every 2metres with high visibility tape or flagging. Steel banding and plastic
banding or common rebar will NOT be used as top rails or midrails.

c. Full Body Safety Harness/Lanyard System


General Requirements
Employees will only use full body safety harnesses and lanyards that are approved by
CV. Dira Utama Sejahtera.The full body safety harness and lanyard will be attached so
that an employee cans neitherfree-fall more than 1.8 meters nor contact a lower level.

This covers the risk of falling over orinto deep water, falling into rotating machinery or
any other hazards that can be eliminated.
The system will consist of a full body safety harness with 2 shock absorbing lanyards or
ashock absorber with two lanyards in a Y arrangement. A shock absorber is mandatory
in any CV. Dira Utama Sejahtera harness and lanyard system.
The two shock absorbing lanyards will be attached to the D-ring of the harness located
inthe middle of the back.
The free end of each lanyard will be supplied with double locking type snap hook.Work
positioning lanyards/devices are to be attached to D-rings at the waist belt
locationand be supported by an appropriate work belt. The positioning lanyard must
always bebacked up by a properly secured shock-absorbing lanyard attached to the Dring located atthe middle of the back.
When using a beam or railing as fall protection anchorage points (if each supports
2.25tonsper person), a beam strap will be used to wrap around the beam or railing,
and the lanyardwill be attached to the ring on the beam strap. Alternative devices to
provide attachmentpoints are authorized if they withstand a 2.25tons shock load.
Note: If an employee is force to reach or position any part of their body beyond the
plane of the structure boundaries a full body safety harness should be used to ensure
100% fall protection and to comply with our requirements.
Only approved full body safety harness may be used on the CV. Dira Utama Sejahtera
operational sites.
When and Where Required
Employees will wear full body safety harnesses while performing work at other than
ground or solid floor elevations and when other safeguards, such as standard
guardrails, nets, planking, or scaffolding cannot be used to protect employees from a
fall distance of 1.8 meters or more.
Full body safety harnesses must be used above 1.8 meters under the following
conditions:
-

When working on floats, or other suspended scaffolding or working


platforms, the suspension of which is by ropes or any means where the
support for the working platform is suspended from above.
When working or walking; from a man basket, painter's stage, or temporary
staging of a similar nature (width, move ability, etc.), even though hand
railed.
When working or walking on elevations and on surfaces (platforms, pipe
racks, open structures, etc.) where the possibility of a fall exists.
When working or walking: (horizontal and vertical surfaces) with an
unprotected side or edge 1.8 meters or more above lower elevation and
standard guardrails are not in place.
When engaging in leading edge work 1.8 meters or more above lower levels
and guardrails are not in place.
When receiving material at a hoist area where the guardrails have been
removedand the employee must lean through the access opening or out

over the edge. Thefall arrest system will allow the employee to go only as far
as the edge of the hoistway.
When working on, at, above, or near wall openings (including those with
chutesattached) where the outside bottom edge of the wall opening is
2metres or moreabove the lower levels and the inside bottom edge of the
wall opening is less than1metre.
When working or walking on surfaces that have holes more than 1.8 meters
abovethe lower level and are not protected by a hole cover or standard
guardrails.
(A holecover or guardrail system will be the primary fall protection but may
need to beremoved because of construction/maintenance activities.) When
this is takes placethe correct procedure for marking and identifying the
location should be documented by the CV. Dira Utama Sejahtera supervision
and barricade at all times.
When working on the face of formwork or reinforcing steel 1.8 meters or
more abovelower level.
When working from an aerial lifts or approved personnel hoistk. Working
next to a well, pit, or shaft 2metres or more above lower levels and the
useof standard guardrails as the primary fall protection system is not
feasible.

Core and Use


-

Softeners will be used where lanyards contact sharp edges.


Full Safety Body Harness and lanyards will not be dragged over concrete or
rough surface.
Full Safety body harness and lanyards must be thoroughly dried out after
they become wet, otherwise, deterioration will be hastened regardless of
the fiber.
Full Safety body harness and lanyards will be dried before storing
Full Safety body harness and lanyards will not be stored on the ground or on
concrete floors but rather hung on pegs or hooks on dry walls.

To maintain the existing strength of anybody safety harness or lanyard, it will be


stored safe from harmful fumes, heat, chemicals, moisture, sunlight, rodents, and
biological agents. Full body safety harnesses and lanyards will be stored in a dry place
where air circulates freely about them. Full body safety harnesses and lanyards will
not be stored unless they have been cleaned. They can be hung in loops over a bar or
beam and then sprayed with water to remove dirt. After washing, the lanyard will be
allowed to dry and then be shaken to remove the rest of the dirt. Lanyards must be
attached to the lanyard clip on the full body safety harness when not in use. This will
keep the lanyard from dangling and hanging loose.
Lifeline System
Vertical Lifelines
Vertical lifelines will be able to support, and be attached to, an anchorage point that is
capable of supporting 2.25 tons per person. Only one person will be attached to a

vertical lifeline fall protection system. Vertical lifeline systems used for fall protection
on suspended scaffolds will be secured to an anchorage point that is independent of
the scaffold and support system. Vertical lifelines must be used with approved rope
grabs for lanyard attachments.
Horizontal Lifelines
All horizontal lifelines placed in skeletal steel structure (i.e., pipe racks etc.,) will be of
12 mm wire rope. The horizontal lifeline system will be designed, installed, and used
under the supervision of a designated competent person and as part of a complete fall
arrest system, which maintains a safety factor of 2.Anchorages used for attachment of
personal fall arrest equipment will be independentof any anchorage being used to
support or suspend platforms and capable ofsupporting at least 2.25 tons per
employee attached, or will be designed, installed, andused as follows:
As part of a complete personal fall arrest system that maintains a safety
factor of 2(two).
Under the supervision of a designated competent person

Lifeline Placement/Installation
-

Lifelines will be installed by designated competent person(s).


Softeners will be placed where the lifeline contacts sharp edges.
Intermediate supports will be adequate to minimize sag and vertical
deflection under loading.
Lifelines will be arranged to provide adequate mobility and maintain fall
protection for employees.
Lifelines will be installed to provide a shoulder high tie-off point for
employees using them.
Employees installing lifelines will be protected from falls by use of a
retractable lifeline or tie-off to structural steel, etc.

Care of LifeLine
To maintain the existing strength of any lifeline rope (fiber or wire) it will be stored
safe from deleterious fumes, heat, chemicals, moisture, sunlight, rodents, and
biological attack. Lifeline ropes will be stored in a dry place where air circulates freely
about them.
d. Safety Nets
Safety nets system may be used as a secondary fall protection methods when it is
found tobe unfeasible or impossible to use other forms of secondary fall protection. It
will beprovided when the use of ladders, scaffolds, temporary floors, or other fall
protection is impractical. The CV. Dira Utama Sejahtera HSE Department/ HSE
Coordinator will be consulted before usingnets as fall protection.
Safety net systems and their use will comply with the following provisions:
Safety nets be installed as close as practical under the walking/working
surface but in no case more than 9 meters.

Safety nets will be installed with sufficient clearance under them to prevent
contact with the surface or structure below.
Safety nets will extend outward from the outermost projection of the work
surface asfollows:

Safety nets will be drop tested with 18kg. If testing is not possible, the net
will be certified by a designated competent person that the installation is in
accordance with applicable standards.
e. Covers
Covers for holes in floors, roofs, and other working surfaces will meet the following
requirements.
-

Cover in roadways will be capable of supporting twice the maximum axle


load of the largest vehicle expected to cross over the cover.
All other covers will be able to support without failure, twice the weight of
the employee, equipment, and material that may be imposed.
All covers will be secured to prevent displacement, and they must extend
adequately beyond the edges of the hole.
All floor hole covers 30cm in length and 23cm in width or less will be marked
with Hole or Cover.
All floor holes covers greater than 30cm in length and 23cm in width or
diameter must be identified with a sign reading HOLE COVER. DO NOT
REMOVE.

Special Circumstances
Special fall protection systems/procedures may be required when engaged in leading
edge work, precast concrete erection work, bricklaying operations, or roofing work on
low-slope roofs.
The CV. Dira Utama Sejahtera HSE Department/HSE Manager must be consulted and
approve any fallprotection plan that incorporates the use of a warning line and safety
monitor system or a Controlled Access Zone.

Inspection Procedures

General Requirements
New body safety harnesses, lanyards, and lifelines will be inspected thoroughly by a
designated competent person to determine that no parts are damaged or defective
before being put into service.
Before using a body safety harness or lanyard, and before installing a lifeline, the
employeewill visually inspect the harness, lanyard, or lifeline for any defects.Under no
circumstance is a body safety harness, lanyard, or lifeline with a known defect
orexpired inspection date to be used.
An inspection of ALL body safety harnesses and lanyards must be completed before
initialuse and at least monthly. This inspection is to be performed by a designated
competentperson who has been trained to use this procedure and the manufacturers'
recommended inspection procedure. A report containing the date, serial number, and
results will be maintained on file by the Safety Officer.
Lifeline systems will be inspected weekly by a designated competent person. This
inspection will be documented.

Full Safety Body Harnesses


Inspection of full body safety harnesses will be done thoroughly, paying close
attention to the following items:
Brads
Buckles
D-rings
Webbing (must be free of frayed, cut, or broken fiberscheck for tears, abrasion,
mold, burns, discoloration, etc.).
Stitching (pulled or cutbroken stitching may indicate impact loading).
Broken strands.
Condition of grommets and buckle (make sure buckles work freely).
Presence of manufacturer's date, tag, and serial number.
Harnesses and lanyards continually exposed to chemical atmospheres for over one
week will be closely examined.
Items of harness hardware must not be damaged, distorted, broken, or have
corrosion, worn parts, burrs, or cracks.
Lanyards
Inspection of lanyards will be done in the most thorough manner. Look at every inch of
the lanyard. Rotate the lanyard so that the entire circumference is checked. Spliced
ends are to be checked very closely.
Hardware
Hardware must be thoroughly inspected and the following precautions taken:
Locking Snap hooks. Check for distortion, cracks, pitted surfaces, and corrosion. Assure
that the latch seats properly and the spring exerts sufficient force to close the latch.
Check to make sure the locking mechanism is properly working.

The thimble must be firmly seated. The splice will be free from cut strands or loose
ends.
Rope Lanyard
Observe the number of fibers broken on the outside.
Open up the rope by untwisting the strands. The interior should be as bright and clean
as when it was new. In manila rope, an accumulation of powder-like sawdust indicates
excessive internal wear.
Displacement of yarns or strands, variation in size or roundness of strands,
discoloration, and rotting are indicators that the rope should be taken out of service.
Spots where the rope has been exposed to oils or chemicals indicate an unsafe rope.
Rope lanyards that have been continuously exposed to chemical atmospheres for over
one week will be closely examined.
The "fingernail test" is a quick test for chemical damage. A good estimate of the
strength of fibers can be made by scratching the fibers with a fingernailfibers of
poor strength will readily part.

Web Lanyard
Bend the webbing over a pipe or mandrel and look at each side of the webbed
lanyard. Bydoing this, cuts and breaks will be revealed. Any swelling, discoloration,
cracks, or charringare obvious signs of chemical or heat damage. Stitching will be
observed closely for anybreaks.
Note: Due to the motion or slippage on a supporting surface when under high
tension,synthetics sometimes melt on the surface and form a skin. This may
be evidence ofdeterioration.
g. Training Requirements
CV. Dira Utama Sejahtera will provide specific training to each employee that might be
exposed to a fall hazard. The training will enable each employee to recognize the
hazards of falling and the procedures to be followed to minimize these hazards. All
training must be documented.
The Project Manager/Site Coordinator and Safety Officer will assure that employees
have been trained, as necessary, in the following areas:
Nature Of Fall Hazards In Work Areas.
Correct procedure for erecting, maintaining, disassembling, and inspecting the fall
protection system to be used. The use and operation of the applicable fall protection
system to be used (i.e., guardrail systems, safety harnesses/lanyards, retractable
lifelines, etc.)
Specialized fall protection procedures (safety monitor, warning lines, controlled access
zone, etc.), if used. Limitations of mechanical equipment use during performance.
Correct procedure for handling and storage of equipment and materials and the
erection of overhead protection.

The Role Of Employee In Fall Protection Plans.


All personnel wearing a full body safety harness must be trained in the use and care of
the equipment.
Pipe rack and skeletal steel
Scaffolding and ladders
Use of lifelines during work
Use of anchorage devices
Floor and wall opening
Excavations
Vertical walls (reinforce steel work , concrete form works)
Equipment crane booms and maintenance (assembling of cranes can create
hazards to the employee). All Contractors and Subcontractors working on the DUS
project site will comply with the above minimum requirements at all times.
Contractors /Subcontractors are also to include in this training the wearing use
and maintenance of harnesses and lanyards as per manufactures recommendation
h. Certifications
A written certification of training will contain:
Name and ID number of the employees trained
Date(s) of training
Name and signature of the person conducting the training
Subject of training
The records of training will be maintained for the length of the project, plus 7 years,
and will beavailable for inspection by corporate personnel.
Retraining
Employees will be retrained on an annual basis or when:
Changes in workplace render previous training obsolete
A change in the type of fall protection system or equipment occurs
The affected employee has not retained the necessary understanding of the
fallprotection system or equipment
Training will also take place when the observation by the DUS management
and Safety department of personnel not using the correct method in the
execution of the work
If a fatality takes place
