Date/Time of incident: 5 Sep 15, 0059 hours

Date/Time of blue alert: 5 Sep 15, 0500 hours

Location: 3920 Clarke St. The location is a single-story residence located in the rear of
3924 Clarke St. It is accessible via two walkways from Clarke St. One was a wooden gate

west and a rear patio door facing north. It was enclosed on the south and east by low fences
(less than 6). Aside from the walkway, it was enclosed by buildings and high fences (over

Note: The location is referred to as 3916 and 3924 in some transmissions and
supplementals. The Citys web-based parcel locator has 3920 and 3924 reversed. I visited
the incident location and confirmed the addresses marked on the residences before
completing this report.
