Measles vaccination coverage at national level also declined from 95% in 2005 to
87% in 2006. Figure 4.9 shows regional variations in measles vaccination coverage
and that in regions the coverages were lower than 80%. These regions are
Kilimanjaro (74%), Pwani (73%), Dar es Salaam (73%) and Kigoma (76%).

A total of 62,100 tuberculosis cases (all forms) were notified in 2006 compared to 64,200
in 2005. The number of cases in 2006 was less than that of 2005 by 2,100 cases (or
3.3%). This was the second consecutive year when the programme notified fewer cases
than the previous year. Re-treatment cases were 4,635 (or 7.5%) of the total notified
cases in 2006. The notification rate for all forms of tuberculosis was about 164/100,000
population which is 48% of the WHO estimate of 342/100,000 population.
The total number of pulmonary cases among the new cases was 44,844 (or 72.2%) and
12,621 (or 20.3%) were extra-pulmonary cases. Similarly 24,724 (39.8%) of all the new
cases were smear positive, giving a notification rate of 65.4/100,000 population which is

As in the previous years, most of the TB cases were notified from about one third of the
regions in the country. Regions with large urban settings such as Dar es Salaam and
Arusha have the highest number of TB cases notified. Figure 1 below shows that 11
regions together notified more than three quarters of all the TB cases in the country in
2006 with Dar es Salaam alone contributing almost one quarter. Most of these regions are
in the eastern part of the country as Figure 6.1. shows.

A total of 4,635 re-treatment cases were notified in 2006, representing 7.5% of all patients
notified. This represented a decrease by 397 cases compared to 2005 when 5,032 patients
were notified. Most of the re-treatment cases were in the categories of others ( 2,441) and
relapse 1,817. The categories of return to control and failure were 257 and 120 cases
respectively. The total number of patients in the failure category decreased by 15 cases
compared to 2005 and in the categoty of return to control there was decline of only 2 cases.

Out of the 25,269 smear positive cases notified in 2005, 19,872 (78.9%) were cured and 891
(3.7%) cases completed treatment, which is 82.6% treatment success rate. The treatment
success ranged from 69.5% in Kilimanjaro region to 93.5% in Mtwara region. Those who for
various reasons did not complete the treatment were 4,506 (17.8%) those who died while in
treatment were 2,394 (9.5%) and 2,112 (8.4%) had unfavourable outcomes (failure, out of
control and transfer out). Ruvuma and Iringa regions had the highest death rate (21.6%) and
(20%) respectively, while Kilimanjaro region had the highest unfavourable outcomes (18.6%)
and Mbeya the lowest unfavourable outcome (1.9%). Nonetheless, the national figure of the
combined unfavourable outcome is within the WHO recommended level of not more than 15%
of the overall treatment outcome. This is good performance despite the relatively large number
of patients notified and the generalised HIV epidemic in the general population and among TB
patients. As for smear negative and extra-pulmonary TB cases notified in 2005, overall 84.4%
completed their treatment while 9.9% died during treatment and 5.7% had unfavourable
outcomes. This compares favourably with results of the smear positive cases notified in 2004.

Among the 5,032 re-treatment TB cases notified in 2005, 1,900 (37.7%) cases put on treatment
regimen II were cured and 1,979 (39.3%) completed treatment, giving a treatment success of
77.0% . The unfavourable outcomes 638 (12.7%), died 27(0.5%), father 208 (4.1%) out of
control40 (0.8) and 240 (4.8%) were transferred out. Regions reporting the failure cases were:-
Kinondoni (3), Morogoro (3), Temeke (3), Kilimanjaro (3), Pwani (3), Ruvuma (2), and one case
each for Dodoma, Mara, Mbeya and Shinyanga.

Leprosy cases notified with disability grade II for 2006 were 374 cases or 11.2% of all new
leprosy cases notified. The percentage of disability grade II notified in 2006 was higher
compared to 2005 when only 9.6% cases had disability. Of those with grade II disability,
336 (87%) are MB cases, 51 (13%) are PB cases. The number of children age less than 15
years who had disabilities grade II were 5. Ranging from 0% in Manyara to 39% in Mara
region. The population of patients with grad II disability varied widely among regions.

There were 3,448 cases of leprosy registered at the end of 2006, giving a prevalence of 0.9
per 10,000 of the population. The registered prevalence in 2005 is 1.1 per 10,000 of the
population. This is equivalent to a decline of 18.2% in the registered prevalence compared
to the previous year. Of the registered cases, 2,730 (79.2%) were MB cases and 718
(20.8%) were PB cases. d children less than 15 years were 208 (6%) of the registered cases.
Furthermore the data indicates a wide variation in the registered prevalence in the country
ranging from 0 per 10,000 of the population in Arusha to 3.6 per 10,000 of population in
Mtwara region.

Out of the 3,731 MB leprosy cases notified in 2004. 3,322 (89%) completed their treatment
successfully. The cases that died while still on treatment were 34; the cases transferred out
to other regions were 43 and 205 (5.0%) cases defaulted while on treatment. The
completion rate in 2004 was at 89% less than in of 2003 which was at 91.5%. Table 7.3
below summarizes treatment results of MB cases notified in 2004.

Of the 1,407 PB leprosy cases notified in 2005, treatment was completed for 1,371 cases
(97.4%). The completion rate in year 2005 (97.4) was higher than in 2004 (95.5%). Two
cases died while still on treatment and 6 others were transferred out of their region while
28 (2.0%) cases defaulted treatment. The general trend of treatment completion for both
MB and PB categories in the country show notable improvement since 1993.

Medicine plays a very important role in the delivery of health care in Tanzania. Availability of
medicine in health facilities is one of the factors that motivate people to visit them. It is therefore
necessary to maintain uninterrupted supply of these items at the service delivery points at all
times.

The National Medicine Policy (NMP, 1991) and Pharmaceutical Master Plan (1992-2002) are

critical in guiding developments in the pharmaceutical-sector. Both documents are now being
reviewed in order to address current developments in the sector such as The shift from push to
pull supply system, promotion of local manufacturing and new malaria treatments e.g. ALu.
Implementation of the NMP and the Pharmaceutical Master Plan is coordinated by the
Pharmaceutical Services Unit (PSU) at MOHSW.
The main function of PSU is to ensure that the NMP is operated according to the requirement of
National Health Policy with its key roles being to ensure Ensuring that MSD performs according
to the MSD Act of 1993, that adequate funds to procure medicines and related medical supplies
are provided to MSD , assisting health facilities with capacity to determine medicines
requirements, establishing effective strategies for improving the rational use of medicines ,
through Tanzania Food and Drug Authority (TFDA) to ensure the quality of medicines, establish
effective drug management and monitoring systems and at health facility level to reduce drug
waste and pilferage and ensure an appropriate allocation of resources to health facilities for drugs
that takes into account equity, patient load, morbidity and medicine needs.

The 1991 National Drug Policy had an extensive section on Hospital Therapeutic Committees
(HTC), their composition, role and functions and on the need to support them from the national
level through the National Therapeutic Committee and regular meetings. Some of the key
functions of the HTC are in the selection, quantification and procurement of drugs and
consumables, monitoring of adherence to STG, ensuring rational drug use, and monitoring drug
usage/wastage. PSU aim is to provide training to HTCs at hospital, district and regional level, in
the whole country to make them effective and functional.

Staffing and operational capacity in PSU is however limited. Most posts for pharmacists at
regional and district level are vacant. Due to limited resources, the PSU is unable to function
effectively and the shortage of pharmacy related staff in the regions and districts incapacitates
PSU from carrying out its tasks. Tanzania has about 686 pharmacists 352 pharmacy technicians
(PT) and 312 pharmaceutical assistants. There is a serious lack of pharmaceutical staff in at
health facilities and at district and regional levels. Initiatives are being made to increase the
number of pharmaceutical personnel.
