KASUS 1
A middle-aged couple presents to my clinic with an older dog to discuss therapy options for
an oral tumor. The man stands across the exam room, arms folded across his chest. It's clear
that he doesn't like what he's hearing. I notice his skepticism and address it directly. "It seems
you may have some concerns with what I'm describing," I say. "Can I ask what your thoughts
and concerns are?"
This simple question leads to the descovery that the client had a similar cancer himself and
went through many of the procedures I was describing. Understanding his
experiences and worries, I address in more detail how these things are
different in dogs and provide more information along those lines.
By the end of the visit, the clients not only pursue multiple treatments for their pet, but the
man is now standing next to me, chatting and smiling.

A 10-year-old Yorkie died unexpectedly after receiving the third mitotane (Lysodren) tablet
you dispensed to treat her Cushings disease. You know that either you or your room
technician told the client about the possible risks and complications of this treatment because
someone always does.
Now you are being investigated by the state veterinary medical board, your license is at stake,
and Ms. Loveherpet is threatening a lawsuit for $100,000 in damages for emotional distress
for the loss of her best friend Gigi.
Even worse, shes threatening to trash your practice by starting a website to determine how
many other pets you have killed. You couldnt sleep last night because all you could think
about were the 14 years of your life youve spent building a practice that is now at risk
because of a simple breakdown in communication.

The patient is an eightyear old mixed breed terrier/lab male named Albert.
Albert was diagnosed with advanced gastric adenocarcinoma following several
diagnostic tests including blood tests, upper GI radiographs, ultrasound and
biopsy. The treatment options consist of insertion of a feeding tube, and
chemotherapy or euthanasia. Your recommendation is euthanasia due to quality
of life issues. Albert has already lost approximately 15 pounds in the last 2
months during the time of diagnostic workup. The feeding tube would provide
the nutritional needs and would require discipline and time on the part of the
owner. However, it is clear that Albert is in some pain with the rapidly growing
tumors and prognosis is poor, with possible 24 months.
Alberts owner, Helen, is a 38 year old divorced women with two children, a 10
year old daughter and a 7 year old son. Albert came to live with the family during
the time of her divorce five years ago, after the family discovered him at the
local animal shelter. Her children immediately fell in love with him and are very
attached to him. In fact, the family recently had a family portrait done for their
holiday card and Albert was included.
Helen has brought Albert in for annual visits and occasional kenneling when the
family has gone on vacations. As a result, the staff at the veterinary clinic has
also grown fond of this longhaired, scruffy, goodnatured dog.
