As per Indian Medical Laws a valid prescription should have the following inform
ation :

Name, qualification, address and registration number of the doctor


Name, age and gender of the patient
Date of consultation
Name of the Medication prescribed
Directions for use including dosage, frequency and duration
Doctor s signature and stamp.
Refill information

In case the patient needs to take the medicine for a long period of time, it sho
uld be mentioned that a refill can be issued using the same prescription for up
to a specified duration.
HOW TO DECODE YOUR PRESCRIPTION?
You may have come across some abbreviations written after a medicine name on pre
scriptions. They usually specify frequency of use. The commonly used ones are :
OD: Once daily
BD: Twice a day
TDS Three times a day
QID: Four times a day
SOS: If required/as required
STAT: Single dose
So next time a doctor writes you a prescription, don't just wait for his instruc
tions but go a step further and ask him questions about your medications and oth
er aspects of your treatment.
