Welcome to CDMs peanut-free facility! The first day of classes is: Monday, September
12th, 2016. We have an exciting school year planned for you. This booklet is filled with
important information about the operation of our school. If you have any questions or
concerns, please speak to one of administrators in the office at your earliest
convenience:

WAIVER: Every parent or guardian is required to sign our waiver. If you registered online,
you have done this already by checking the required box. If you registered in person, you
should have signed a hard copy waiver. You can download a waiver form from our
website>registration tab> how to register then click waiver. If you are unsure, please
ask at the reception desk.
COMMUNICATION: We request that you keep all communications for the school year as
you receive them from our office. Information regarding our fee schedule, holidays and
cancelled classes are on the following pages and will also be posted on our Google
calendar right on our home page: go to www.cdmdance.com. Our primary method of
correspondence is by email, so please be sure to provide us with your most updated
email address. You can do this by logging into your online account. Please keep the
square indicating, YES, I agree to receive emails checked on your account. We ask
that you check your email at least twice per week. If you do not have email access,
please notify us so we can find an alternate method of communication between CDM
and your family. You will also be expected to read the important information that is
posted on the CDM Information board in the school lobby. At times, we may also send
home printed information. To find out the latest information and released schedules,
regularly check our website, as well as access your online account. Please refresh your
screen for latest website updates. Information for CDM students and families is located
under Existing Students on our website.
LOBBY, VIEWING AND WIFI: CDM offers a comfortable lobby as a place to wait for your
child during lessons. The code to the door at the bottom of the stairs is Schlage 1-5-1-5.
This lock helps keep studio secure from strangers. Please avoid removing shoes and
leaving them on the floor at the bottom of the stairs where they become tripping
hazards. Use the shoe racks provided. We give you the opportunity to view all classes by
either a viewing window OR a video monitor. We also provide seating for those waiting
to pick up their child or students waiting for their next class. Please clean up after yourself
if you choose to snack in the lobby. Only water or tightly covered drinks and dry NonPeanut snacks are allowed. We encourage you to enjoy the shopping in the plaza when
the studio is at a peak during busy times to avoid unnecessary congestion. The television
may be watched provided it does not disturb others. The remote control is kept at the
reception desk. Just ask us. We offer free wi-fi at CDM. Our network is danceCDM and
password is lovedance.
WAITING IN THE LOBBY: We ask that you respect the activities of others and the classes in
session. There is no running permitted in the hallways and no acrobatics in the lobby.
Small children waiting with parents must be closely monitored. If you use our books
magazines and toys, we would appreciate it if you would return them to the magazine
rack or toy area. Your courtesy with this will be greatly appreciated.
BEFORE AND AFTER CLASSES: Students are requested to arrive to the studio with adequate
time to prepare for class, parking, changing, putting up hair up properly, restroom, etc.
For Parents picking up young children: please be available in the waiting room to
receive your child when dismissed from class. Although we have their safety in mind, we
do not have extra staff on duty to care for young children between or after classes. All
Parents: please make sure to tell your child they must not leave the lobby area or go
through the lobby door until they are picked up. Also, let us know if you will be having a
visitor with you within the premises.
We welcome your comments and suggestions regarding any of our policies or
procedures at any time. Feel free to send us an email or talk to someone in the office so
we can address your concerns. If any parent/student wishes a private conference
concerning your individual dance goals or financial concerns, please do not hesitate to
ask for an appointment with us so we may assist with your personal needs. We are
happy to work with you to ensure that your school year will be one of pleasure and
growth. The entire faculty and staff wish you a fantastic year of dance!

A $40 non-refundable registration fee per family is required upon registration.
For Late registration a non-refundable fee of $10 will be applied per family ($50
registration fee total) for students who enroll after the first day of their class. You will be
given a pro-rated amount for classes missed up until your registration date.

Our $40 Registration Fee is due with September tuition. New students joining after
September will have their class charges pro-rated from the time they join at the
rate posted on the current posted fee schedule and a late registration fee of $10
in addition.
Everyone will have 36 classes over the ten months. Recital and Photo days are
counted in your tuition, as we do not charge a separate recital participation fee
or photo shoot fee for photographers. There are also rehearsals at the end of
each photo shoot and onstage before recital.
Those students joining in Term 3 will not be refunded for classes missed for Photo
Day or trying on costumes day. There will be no refund for those who choose not
to take part in recital or photos. No exceptions.
Late Payment Fees:
Outstanding payments are subject to 2% late charge 14 days from the initial
payment due date. After 30 days from the initial payment date, another 2% will
be charged. After 45 days, CDM reserves the right to prohibit students with
outstanding fees from attending classes until full payment is received.
NSF cheques are subject to a $25 charge.
If your credit card information has changed, please notify the office before the
next payment deadline.

We require 4 x post-dated payments at time of enrolment. Any outstanding
payments owed from the previous year must be paid before re-registering.
Payments should be dated as follows: (students 6 & under are exempt and may
pay for 1 term)
Term 1 tuition fee + HST + $40 registration fee
Recital costume deposit $50
Term 2 tuition fee
Term 3 tuition fee
Recital costume balance payment
Alternate fees that may incur: RAD exam entry and coaching fees, character skirt rental,
photography, recital DVD, dancewear.
Code of Ethics
We the dancers, faculty and staff of Conservatory of Dance & Music, are a
vibrant and caring community of people who have a high regard for each
other, the art of dance, all art forms, and the respectability and value of the
work we do. All who participate are requested to demonstrate the willingness to
be held accountable to uphold the values and ethics we declare as the
fundamental guidelines stated below. In all of our endeavors to study and
perform as an expression of our love of dance and the arts, we show respect to
the parents, our peers, the staff and faculty of Conservatory of Dance & Music
and all of the wider dance community, including our other schools and studios in
the industry including those at competitions. We consider it a privilege to
participate in a worldwide community for the performing arts exemplifying the
values of integrity, character, leadership, responsibility and professionalism. We
are all expected to honor CDMs Code of Ethics. Failure to do so may result in a
meeting with a member of staff, studio manager or director. CDM reserves the
right to end contracts with any one refusing to honour the code of Ethics.
Dress Code
Although we encourage students to express themselves, it is important for
students to abide by our dress code. We have a dress code policy for the
following reasons:
Teachers must be able to see the dancer's body outline clearly in order to
make proper corrections on posture, alignment, etc.
A bun is required for all ballet classes. Hair must be properly secured and
tied up off of the face and neck for all styles of dance. Wisps or stray
hairs should be sprayed flat with hairspray. If you have difficulty making a
bun, please attend our information/welcome meeting in September to
learn how or ask us how in the office. You may also visit the following link

No loose fitting pants, blue jeans or daywear shirts, swimwear or bikini tops
(they are not dance attire).
BALLET
Tots Movement & Music, Creative Dance, Pre-Primary, Primary Ballet,
Girls: Any cotton tank-style leotard *Mondor Tank style #1645 in 'true pink' or Motionwear
($21-$30). Body Wrappers footed C80 colour 'LPK' ($18) and pink full-soled leather ballet
slippers, ex.Bloch Bunny Hop ($22-$25). Short ballet skirts are permitted (not wrapskirt). NO Underwear to be worn under tights, LONG HAIR MUST BE SECURELY TIED UP,
FRINGE SHOULD NOT COVER FACE Hair must be in a bun. No Ponytails for Pre-Primary &
Primary.
Boys: T-shirt blue or white, Navy or black poly-cotton or lycra shorts, White socks, White or
Black canvas or leather ballet shoes ($35). *Prices are approximate FYI w/o HST
Grades 1 & 2
Girls: *Leotard in Royal Blue Capezio CC202, Body
Wrappers Tights C80 BAT or C31 BAT, ($18) Pink leather fullsoled ballet slippers (Bloch #205). No Dance skirts
permitted.
Character skirt (Required for Exam Candidates): Circular,
mid-shin length 3 stripes of 1/4' ribbon around base of skirt
in different Royal Blue and/or Greens to match leotard.
Character shoes, Bloch S0315G 1 heel. Skirts are also
available for rent first come first serve $40. Students, who
do not wish to take an exam or only take x1 ballet class
per week are permitted to wear any circular skirt mid-shin
length with black dress shoes or anything similar.
NO Underwear to be worn under tights, NO BARE LEGS and LONG HAIR MUST BE
SECURELY TIED UP, FRINGE SHOULD NOT COVER FACE Hair must be in a bun. No
Ponytails. No large holes in tights.
Boys: Black tights ($24) or dance pants, black or white solid color T-shirt, and black fullsoled leather ballet slippers. *Prices are approximate FYI w/o HST
Grades 3, 4 & 5
Girls: *Leotard in Forest Green, Capezio CC202,
Footed/Convertible Tights Body Wrappers C80, C31, A31
A80 BAT ($20). Pink leather full-soled ballet slippers (Bloch
#205). No Dance skirts permitted. Character skirt: Circular,
mid-shin length 3 stripes of 1/4' ribbon around base of skirt
in different Royal Blue and/or Greens to match leotard.
Character shoes, Bloch #314 RAD-approved higher-heel
($42) NO Underwear to be worn under tights, NO BARE
LEGS and LONG HAIR MUST BE SECURELY TIED UP, FRINGE

Boys: Males white short-sleeved dance leotard OR well-fitting white Tshirt, Black or Navy
poly-cotton shorts or tights, White socks, Black or white canvas or leather ballet shoes
($30-$35). A dance belt is required under shorts/tights for added support. Black, leather
character shoes dress shoes or tap shoes with the taps removed. *Prices are approximate

Girls: Black leotard. Black dance shorts. Bare feet or *Bloch 'Foot thong 2 or 3'.Kneepads
are permitted when necessary. LONG HAIR MUST BE SECURELY TIED UP, Hair must be in a
bun. No Ponytails. Fringe should not cover face. All undergarments and straps (if
necessary, should be concealed under the leotard.
Men/Boys: Black tights or dance pants, black or white solid color T-shirt. Dance belt.
Students dance in bare feet or open modern sandals. Kneepads are permitted for
floorwork.
TAP
Girls: *Black tank style leotard or tight fitting top, tan convertible
tights and/or Black dance shorts. Ages 6-12: Capezio Mary Jane
style Black tap shoes #3800 ($55). Advanced tappers: Black Bloch
Showtapper ($75 ). No split sole tap shoes please & DO NOT BUY
TAP SHOES FROM WALMART or PAYLESS LONG HAIR MUST BE
SECURELY TIED UP Ponytails are allowed. No Bare Midriffs. Blue
jeans are not allowed.
Men/Boys: Black tights or dance pants, black or white solid color T
shirt. Dance belt. Black tap shoes Capezio Mens Tapster ($50).
HIP HOP
Girls: CDM Sweat Pants and T-shirts are available at our reception
desk boutique. We highly recommend them for Hip Hop Class!
CDM sweatpants $32 +HST for children, $42+HST for adult sizes
(15% year round for hip hop students)Any color leotard or tight
fitting top, crop top or t-shirt with no logos. Sweat pants, black jazz
sneakers or white-soled sneakers. No bare-midriffs for ages 5-11.
LONG HAIR MUST BE SECURELY TIED UP Ponytails are allowed. Blue
jeans are not allowed.
Men/Boys: Any solid color T-shirt and black dance pants or sweatpants, white-soled
sneakers or black jazz sneakers.
MUSICAL THEATRE
Girls: Any color leotard or tight fitting top, black jazz/yoga pants or dance shorts, black
jazz shoes.
Boys: Any color shirt and black dance pants or sweatpants, black leather ballet shoes
or black jazz shoes.
