with my previous responsibility including their cultures
and values, but being one of the faculties on our school

Philippines has become expensive and costly. Only
few can afford to enroll in prominent universities

and colleges. Graduating high school students find


solace in alternative institutions where lucrative
learning is offered in a minimal tuition fee basis,

Computer and technical schools have sprout in all
the vantage points in the country. This seems an
answer to the demands of the brave-new-world

ENTRANCE REQUIREMENTS
A students registration of Chrisville Institute of
Technology, Inc. automatically binds him to all
rules, regulations and traditions of the institution.
The institution reserves the right to dismiss a
student any time if his scholastic record and moral
conduct prove that he will not profit from the
education offered by the institution.
If the students parents reside outside the city, the
student should have a guardian. The guardian
should be a responsible person duly authorized in
writing
by
the
parents
to
assume
his
responsibilities towards the student and act in
behalf of the parents.
Although the Institution cannot be held
responsible for the students conduct outside the
institution premises, undesirable conduct outside,
as well as inside the premises may be a cause for
dismissal.
If a student disregards the rules and regulations of
the institution by violating them, it may be a basis
for suspension or immediate dismissal/expulsion.
The disciplinary action depends upon the nature
or frequency of the offense.

Requirements for Married Female Students
1. A student in her third month of pregnancy is
required to apply for exemption from wearing
the prescribed uniform.
2. A married student who is pregnant and will give
birth during the semester shall not be allowed
to enroll.
3. A married student who has given birth shall not
be allowed to return before sixty days after
delivery.

Grading System
The mark/grade given to a student is an indicator
of the students mastery of the subject and a
mark of scholarship. Therefore, in determining any
mark / grade, only those factors that afford
evidence of the degree to which the student has
attained the objectives set up for the particular
course or subjects are taken into consideration.

examinations for each subject which will
compose of 60% of their final grade are as

discretion of the respective instructor for any

2. Marks/grades in subject consisting of lecture
and laboratory classes should be the result of

Academic Honors
Students, who complete their baccalaureate
courses with the following grade point averages,
shall be given with honors:

of the honors are used, namely; With Highest

Withdrawal
A student who intends to withdraw from or drop
any subject enrolled in a particular semester, may
do so provided that the official dropping of
subject is made before the Midterm or the 2nd
rating period of the semester. Dropping Forms
are available at the Registrars Office. This must
be fully accomplished before a student is officially
considered as Dropped or Withdrawn from the
roll. Unauthorized dropping may result to a grade
of 5 or Failed in the subject concerned.
If a student intends to drop 3 or more or all of the
subjects enrolled, a parental consent is needed
before the student is allowed to drop his/her
subjects. Failure to do so may disallow the student
from dropping the subject or if the student fails to
report to class, it may result to a grade of 5 or

A student is entitled to a refund of tuition fee,


except the miscellaneous fee, in accordance

Any student who wish to transfer to another


school, college or university, may do so provided
that he has cleared himself of all the liabilities and
responsibilities (administrative, academic and
financial) in the university. The necessary
documents for transfer could be availed at the
office of the registrar.

A student gets an Incomplete Grade (or INC)


once he/she has not completed the requirements
of the subject (e.g. no major examination such as
preliminary exam).

Completion of an incomplete grade shall be made


within one (1) semester after the incomplete
grade was issued. Completion forms are
available at the Registrars Office. This must be
fully accomplished by the student. The student
shall bring the completion form to the instructor
concerned for the completion of the requirements

Students who have already graduated and those


who wish to transfer to another school must
submit a duly accomplish clearance form before

available at the Registrars Office. The student will
have to surrender his School ID to the Office of
Student Affairs before clearance is granted.

LIBRARY
Textbooks, reference books, interest and hobby
books and other periodicals can be found at the
library of the school. The student must present
his/ her school ID upon borrowing of books. The
school ID can be claimed after the book has been
returned.

GUIDANCE AND COUNSELING


This offers various services which can help mold
the wholesome personality of the student. Various
programs are launched by the institute for the
whole school year. This includes but is not limited
to:

Registrar and validated for the current semester.
Rules and regulations regarding the proper use of
the School ID has been set forth by the institute
and student offenders are to be treated in
accordance with the Rules and Regulations.
How to apply for a School ID
1. Present the following:

2. Fill up the Identification Card Slip available at


the Office of the Registrar.
3. Wait for the schedule of picture taking by the
official photographer (this will be posted).
4. Wait for the official list of ID Cards for release
before claiming the School ID.
What to do in case of lost School IDs:
1. Secure an affidavit of loss of ID.
2. Present the following at the Registrars Office:
a. Official Registration Form
b. Affidavit of Loss
3. Pay the required ID Fee plus penalty for the lost
IDs
4. Come back on the date specified for your ID

Students can address their mails at the Chrisville
Institute of Technology especially if the letters are
official in nature. Mails can be claimed at the
Registrars Office during office hours provided that
the student present his/her School ID. It is
recommended that the letter be addressed in the
proper way as presented below:

ID Validation
All the students who are officially enrolled for the
semester are required to have their ID Cards
validated for the current semester. No student will
be allowed to enter the school premises without
the validation sticker even when the student has
officially paid his school fees. Validation stickers
are available at the Registrars Office.

All students are required to come to school in

RULES ON DISCIPLINE

RULES ON DISCIPLINE
I.

GENERAL DIRECTIVES
Wearing of the Prescribed Uniform. Every
student shall wear the prescribed uniforms on

Mondays to Fridays, prescribed PE Uniform during


their PE classes and NSTP T-Shirts on NSTP
classes.
I.D. Requirements. Every student shall wear the
official identification card (ID) at all times inside
the institute.
Student Attendance
1. Every student shall attend the classes promptly
and regularly.
2. Students should come to class early or on time.
A student is marked late if he/she arrives within
15 minutes after the time.
3. A student should incurred 3 consecutive days
late is equivalent to 1 day absent.
4. Student who has been absent for 3 consecutive
days from any of his classes must present a
letter duly signed by his parent/guardian or
must present a medical certificate.
5. Unexcused absences beyond 5 consecutive
times will cause the student to be dropped
from the class lists.
School and Classroom Conduct
1. Students should be inside the classroom on
time. They are obliged to extend respect to
teachers/instructors, school personnel and
visitors who enter the room.

2. Students should always be prepared everyday


in classroom activities, be attentive in class
and should refrain from doing acts which will
disturb the class discussion.
3. Students must be polite in asking questions or
in making appointments with the instructors.
Dialogue after the class should not disrupt
instructors class schedule.
4. Students should always be respectful when
talking to instructors and superiors.
5. Cellular phones should be switched off while
inside the room.
6. Student should help maintain the cleanliness
and orderliness of the room. They should
refrain from spitting anywhere and must make
use of the trash cans to dispose the pieces of
paper and other waste. Vandalism and
destruction of properties are prohibited.
7. In coming in and going out the classroom,
students should observe silence.
8. In case the instructor fails to arrive on time, the
students are required to remain inside the
classroom, observe and maintain silence.
9. Students found smoking and under the
influence of liquor will be sent out and be
subject to disciplinary action.
10. Students should help in the maintenance of
peace and order within the school.

11. No one is allowed to carry fire arms, knives


or other deadly weapons whether licensed or
not, except security personnel in the course of
his duty.

Examination. At the end of each semester an


assessment test in all subject areas is
administered to evaluate the performance and
skill of each student.

12. It is strictly prohibited for any student to


possess, distribute and take all forms of illegal
drugs and/or paraphernalia.

the bulletin board at least two weeks before
the actual date.

13. Students are obliged to read whatever


updates posted on the bulletin board.

2. Accounts should be settled at the Cashiers


Office at least a week before the scheduled
examination.

14. Students must present their class cards to


the Instructor of every subject during the first
week of classes. No class card will mean that
the student is not in official class list.

3. Before the start of the examination, students


must secure examination permit and test
booklets from the office.

Any form of gambling is strictly prohibited.

instructor/proctor for some questions or
clarifications without disturbing the class.

3. Equipment is on a first come, first served basis


and cannot be reserved, Please do not attempt
to reserve a computer by leaving your
belongings around it. If a station is left
unattended for 15 minutes or more, laboratory
in-charge may log you out of the computer and
move your belongings to give way for another
user.
4. Log-in using the Student account.
5. Use of computer facilities should not hamper
nor interfere the ability of the other students to
do academic work. Activities to be avoided
include excessive computer volume, loud
conversation or laughter, use of offensive
language, viewing of offensive or explicit
imagery, cell phone call and other disruptive
behavior.
6. Keep your hands clean. Grease and other gunk
get in between the keys, sticks to disk, etc.
7. Dont eat or drink inside the laboratory.

1. The Computer Laboratory is open to all


Chrisville students who are enrolled for the
semester but can be used only during
scheduled time.
Be aware of your scheduled time slot if it is 1
hour or 30 minutes.
2. Students must at all times wear their proper
uniform and ID within the school premises
especially inside the computer room.
Wearing of cap/hat is strictly not allowed while
inside the computer lab.

8. Dont smoke.
9. Make sure that no paper, book or material
blocks air circulation around any piece or
equipment.
10. Never move the computer while the power
is on not even an inch or two. Even slight
movements can destroy data on the hard disks.
11. You must have a valid user account for you
to use the computer. If you are new to the
school, ask the person in charge at the
laboratory for assistance.

12. You must not use your network account to


log another person into the laboratory even to
your trusted friend or family member. To do is a
violation of the laboratory policy.

19. A user engaged in non-academic activity


may be asked to surrender his computer to
allow other user to take over.
20. Viewing of CD, VCD, DVD, movies, etc. is
prohibited unless instructed by the office.

13. Work quietly and avoid chatting with your


seatmate.

21. For safety, always turn off and unplug your


computer an all peripherals before beginning to
lean the hardware.

14. Be courteous to your fellow laboratory


patrons by turning off your mobile phone ringer
and by taking all personal conversations and/or
cell phone calls outside the laboratory.

22. Turn off the system and pull them main plug
if there are thunderstorms or any other signs of
electrical disturbances.

15. If you have any questions or clarification,


ask your instructor about it

23. Do not leave your personal belongings


unattended. Laboratory assistant is not
responsible for lost, stolen or moved items. It is
your responsibility to keep your own property
safe and secured

16. You must not install software on the


laboratory computers. If you need specific
software for academic purposes, ask the
laboratory assistant to find out the appropriate
procedure for having it installed.

24. Lost and found items are stored for a certain


period in the Registrars Office.

17. Do not configure computers, personalize


desktops or alter another users files.

25. Students are not allowed to bring their own


computer / CPU inside the laboratory.

cables.
If you have a valid reason for wanting to alter a
compute station, ask the laboratory assistant
or laboratory in-charge to find out the
appropriate procedure
18. Never use the INTERNET if you are not told
to do so.

II.

DISCIPLINARY STANDARDS
Categorization of Behavior Based on Gravity
of Offense Committed
Offensive behavior is categorized as either major
or minor offenses. Major offenses include those

behaviors which seriously violate the preservation


of life and property of the institution and violate
the dignity of a college student. All other offensive
behavior not categorized as major offenses are
classified as minor offenses.

Categorization of Behavior Based on Locus


of Responsibility
For clarification as to the locus of responsibility for
disciplinary action, offensive behaviors are
classified into: Institutional, academic and
personal offenses.
Institutional Offenses. These offenses are to be
handled
by
the
School
Administrator
in
coordination with the Department Head/ Course
In-charge of the erring student. Any administrative
personnel or faculty member or student who has
observed violation of the Rules on Discipline
classified as institutional offenses can report such
violation to the Security Office or directly to the
Department Head/ Course In-charge, who
coordinates with the School Administrator in
discussing the case of the student with the
Institutions Discipline Board.
Academic Offenses. Academic offenses are to
be handled by the instructor/ teacher concerned,

investigation conducted, the student is given the


corresponding
penalty
for
the
violation
committed.
Personal Offenses. The approach depending on
the situation and circumstances is to be used in
the discussion of the offense committed by the
student against his co-student.
Behavior Subject to Disciplinary Action
The students right to exercise the freedom to
learn involves freedom from disciplinary action
except through due process. Disciplinary action
against a student should be taken only for
violation of institutional standards of behavior and
regulations which are clearly defined as possible
which represent a reasonable degree of control
over the students.
Fair Procedure in Applying Disciplinary
Actions
On all occasions, the school officials shall exercise
fairness in applying disciplinary measures to the
erring student. This means that the student has
previous knowledge of the standards of the
student behavior in the academic community, that
he be informed of the nature of charges against
him, that he be given a fair opportunity to
disprove them and that there is a provision for
appeal of a decision.
The Students Right to Appeal a Decision

When the misconduct of a student may


necessitates serious penalties and when the
students question the fairness of disciplinary
action taken against him, he should be grated,
upon, request, the right of being heard before a
Hearing Committee duly constituted by the School
Director and approved by the board of regents,
thus:

All pertinent information or evidences must be


presented before the Hearing Committee and
the decision shall be properly recorded or taped
or documented.
The decision of the Hearing Committee shall be
final, subject to the right of the student to
appeal to the School Director through the
School Administrator to the School Board of
Regents.

The Hearing Committee should include the


School Administrator, the Head of the Student
Affairs, Department Head, the student and the
adviser of his choice. No other person/s who
may be interested in the particular case may sit
in judgment during the proceeding.
The student shall be informed in writing of the
proposed disciplinary action taken against him,
with specificity and enough time to give the
student a chance to prepare for the hearing.
The student shall have the right to be assisted
in his defense by an adviser of his choice.
The burden of proof shall rest upon the school
officials bringing the charge.
The student shall be given the opportunity to
testify and to present evidence and witnesses.
The student has the right to question adverse
witnesses and to rebut unfavorable remarks/
judgment made against him and to be told of
the names of those who made them.

Offenses committed by the students outside the


university and outsiders are considered beyond
the locus of responsibility of the Institute.
However, in cases where in both parties involve
Chrisville students, the institute will act on the
case, provided that a formal complaint will be
filed. The case will be dealt in accordance with the
usual procedure.

For purposes of clarifying as to who is responsible


for determining disciplinary action to be meted on
the erring student, offensive behavior is classified
into: institutional, academic and personal.
The Teacher. The faculty member has the sole
responsibility to discipline any student judged
guilty of the offenses classified as academic
(behavior which occur inside the classroom or
school corridors. Should the offensive behavior
leads to penalty, a recommendation for

disciplinary action is made by the teacher


concerned in coordination with the Department
Head to the School Administrator.

The Institutions Discipline Board shall have the


responsibility of passing ultimate decisions of the
case presented.

The Department Head. The Department Head


conducts
investigation
of
the
case
and
recommends re-trial, as the case may be, to the
School Administrator.

The parents shall be involved in the affairs of their


children particularly in the cases of misbehavior.
They shall be immediately notified by either the
Department Head, or School Administrator. During
the scheduled meetings or case conferences, the
parents are to be physically present to show
interest in the case of their son/ daughter.

on the gravity of offense as reported by the
teacher and as determined by due process of
investigating the case through the Department
Head. Offenses within the jurisdiction of the
School Administrator are those which fall under
Academic and Institutional and Personal Offenses.

This
is
composed of the School Director, School
Administrator,
Head
of
Student
Affairs,
Department Head and the Registrar, who shall
constitute the highest governing body for the
discipline of behavior problems of students and is
therefore empowered to pass the ultimate
decision on the case. The student, however, has a
right to appeal to the Board of Regents of the
institution.
