NURS4600 Senior Capstone
Learning Plan
Overview
Completion of the Senior Capstone is done primarily by the BSN student in cooperation with the
course instructor and in most circumstances, a clinical mentor/professional colleague. The purpose of
the plan is to provide a blueprint for learning activities and participation in this learning experience.
The plan is to be completed and approved by the course instructor and a mentor prior to participating in
the Senior Capstone experience.

Statement of Purpose
I am very interested in safety and prevention for patients. Especially in the areas of compliance and
what can be done to decrease risk and multiple incidences. I intend to develop a criteria for fall
evaluation with home health based on criteria for insurance payment, patient need, and fall risk. With a
specific criteria approved by my mentor and thus the hospital I will create and education plan for the
doctors to inform them of the criteria and how to order home health services for home fall risk
evaluations. Within this program I will also look at items that complicate fall prevention such as
compliance with home health suggestions, social isolation, fear of falling and immobility.
Goals
1. Participate in a hospital wide experience based on patient safety/prevention with a large
organization. To gain personal experience in a more administrative and managerial role.
2. Evaluate nursing literature regarding fall prevention practices already in place and success of
preventions.
3. Develop a falls risk criteria for home health evaluations that doctors can refer to. Educate providers
on criteria and ordering of home health services.

Description of Activities
Complete a learning plan
Literature review of fall prevention strategies in place and success of strategies
Consult with Tina Dewey weekly via telephone or in person for feedback and suggestions for
implementation
Present final plan to Tina Dewey
Make changes as recommeded and present for final approval.
Schedule time to educate doctors on criteria for ordering and new ordering process
Participate in 4600 activities as required
Prepare and present experience at class seminar
Track time and submit all required paperwork to instructor on time.

Feedback and approval by Tina Dewey and her supervisor for implementation
Feedback from doctors during education meeting
Success and use of home health fall evaluations for needful patients
