MEMORANDUM OF INVESTIGATIVE ACTIVITY

On May 17, 2016, Senior Special Agent (SSA) Eloy C. Montoya and Management Analyst
(MA) Stacie Abeita, New Mexico Human Services Department (HSD), Office of Inspector
General (OIG), traveled to the NE Bernalillo ISD Office and conducted an interview of
HSD/ISD Manager
. ( JK ) The interview was conducted at NE Bernalillo

ISD Office located at 4330 Cutler Ave NE, Albuquerque, NM 887110. SSA Montoya and MA
Abeita identified themselves to JK verbally and by showing their Office of Inspector General
credentials. The purpose of the interview being conducted was in connection with an official
OIG investigation into allegations HSD/ISD employees are falsifying income information on
emergency applications for clients seeking food assistance.
During the course of the interview with JK SSA Montoya and MA Abeita requested any
ISD cases in which JK may have knowledge in which management directed staff to falsify
client information on applications for clients seeking emergency food assistance. SSA Montoya
and MA Abeita received four emails messages from JK listing four ASPEN case numbers
which may contain client falsified income information. The cases are being reviewed and
analyzed by OIG investigators and results will be reported and documented in future
Memorandums oflnvestigative Activity.

packet timely as client was on TFS, but it closed out and didn't transition to ongoing SNAP. I certified the closure and reapped application for 05/25/2016 when client submitted new application. SNAP is expedite as there is no income in the
