STATUTORY DECLARATION

I, PRAKASH CUMARAKULASINGAM of 384 Kenton lane, Harrow,

Middlesex, HA3 8RG in the United Kingdom DO SOLEMNLY AND
SINCERELY DECLARE as follows:

1. I am the Declarant above-named and I make this Declaration

in support of the application of my mother- in-law to visit to
the United Kingdom to support my wife physically and
mentally as she is suffering from Chronic Kidney Disease.
2. I am a British Citizen and holder of passport bearing number
540130182

3. My mother-in-law have enough saving in her bank account

and she will utilise same for her travelling and other expenses
in UK.
4. I am self-employed and working for Franklyn Lodge Care
Home, 8 Forty Lane, Wembley, Middlesex, HA9 9EB and my
earnings are in the region of 15,000.00 per annum.

5. I am a tenant of the above said property, it is a three bedroom

flat. This property consists of three bedrooms, living room,

kitchen and a family bathroom. The property will be occupied

by me, my wife and four year old daughter Thashna. Therefore
there is ample accommodation for my mother-in-law to stay in
the UK without recourse to public fund.
6. The details of the sponsored person are
MRS. RATHYINDRA DEVI KRISHNARASA (DOB 23-04-1959
passport number N3179683).
7. I undertake to accommodate my mother-in-law in the United
Kingdom without recourse to public funds. I further undertake
to repatriate her at own cost if necessary. I shall meet the
expenses of burial/cremation in the event of her death in this
country.
8. There is now produced and shown to me and hereby exhibited
to this my declaration mark PC1 true copies of my passport,
my wifes passport, my daughters passport and birth
certificate, Bank statements, supporting letter from hospital,
letter from the employer, marriage certificate, my wifes birth
certificate with translation, tenancy agreement and letter from
the landlord.
