Quick Reminders
Before your patient leaves:
Be sure the following have been signed by the Dr. & scanned:
o Lab order forms
o Prescriptions

o Referrals forms
o Any other medical forms

Review your assessment & plan with the attending and then the
patient
Walk the patient to the front office to schedule a follow-up
appointment (let the SIC know when & why)
Please clean up the patient room (change the paper, return
supplies to where they belong, then leave door open and let the
SIC know its ready)


Services:
Meds:
o We have most OTC meds as well as some prescriptions. Please ask
the Lab Coordinator for help on accessing them.

Lab: note- please make sure patient name is spelled accurately

and clearly on the lab form
o
Possible referrals:
o Imaging (limited): X-Ray, Ultrasound, Mammogram, CT
o Specialists (limited dates): Endocrine, Orthopedics, Cardiology,
o Sight Savers (optho), MIND Clinic (Mental Health),
o Diabetes Education/Support Groups (schedule released weekly)
o For all other referrals, please refer to El Rio.
