Greetings!!!
May we request from your good office to defer the release of LTO
DRIVERS LICENSE of JAY DUABE, ____ years old, married, resident of
______and presently employed at _____________. On May___, 2016 at around
4:30 in the morning, he was so drank, recklessly driving his single motor ,
bumped and hit my husband while walking from the market along Sta. Mesa,
Manila , which resulted to serious injuries on the head , neck and to the
different parts of the body, that incapacitated him to work up to this time.
He is only the breadwinner of the family.
Jay Duabe, was very arrogant and doesnt want to settle the damages
sustained. The city prosecutor sent him subpoena for preliminary
investigation but he ignore it.
I sincerely hope that you consider this behavior unacceptable and that
you will take appropriate action to reprimand this driver. Please call me at
