    Dear Sir,
    I am Jaydeep , Working as Account Assistant with Knowledge Consortium of Gujarat
    would like to drawn your kind attention on the matter regarding my absence for the work. I would
    like to request a leave of absence for medical reasons. I will be having hernia surgery on September
    1st 2016 and as per the doctors suggestion after surgery I will be needed 15 days bad rest. I expect to
    return to work approximately three weeks later.
    I can provide written documentation from the surgeon, if necessary.
