Insurance Certificate
This certificate is issued under the authority of the Swedish Legal, Financial and Administrative
Service Agency (Kammarkollegiet).
This is to certify that Mr. Hadush Berhe Asgedom (Date of birth: 01/02/1983), Ethiopia, is
covered by a Swedish State Group Insurance and Personal Insurance GIF, terms and
conditions of insurance 1 July 2007.

Insurance policy number: 202100-4789
The insurance is valid during the period for which Mr. Hadush Berhe Asgedom is taking part
of International Training Programs held by Sida, arranged by NIRAS Natura AB.
The insurance, including travels to and from the training, is valid during the period
October 19 November 3, 2013.
The validity shall contain a period of grace for 15 days.
The insurance includes:

Full coverage of medical costs in the event of medical emergency. No limitation in amount.
Full coverage of costs for emergency dental treatment. No limitation in amount.
Full coverage of costs for medical evacuation or repatriation of remains. No limitation
in amount.
Liability coverage up to 3 million SEK.

The insurance applies:

The insurance applies 24-hours a day in the whole Schengen zone, as well as departure from home country
for direct journey to the Schengen zone up to arrival in home country following direct journey there from
the Schengen zone. When the entire training or a portion thereof takes place in a country outside of the
Schengen zone, the insurance shall apply in an equivalent manner with respect to that country.

The insurance is backed by the full faith and credit of the Swedish government.
There is no deductible to the insurance.
