Copy of Official Receipt of Filing Fee
Exhibits B and C
Accomplished Technical Questionaire, if available
Certificate of Deposited Propagating Materials
All forms and documents should be submitted to the Office of the Registrar of
the Plant Variety Protection Office at the National Seed Quality Control Services
Laboratory, Bureau of Plant Industry, Visayas Avenue, Diliman, Quezon City.
The applicant shall be informed of the sufficiency or lack of it of the application
within five working days from the date the application was received.

It is the responsibility of the applicant to inform the Plant Variety

Protection Office of any information changes in the application forms;
The applicant is advised to refer to the Implementing Rules and
Regulation of Republic Act No. 9168 whenever necessary;
To avoid conflict with variety name or temporary denomination, the
applicant is advised to consult with the Registrar of the Plant Variety Protection
Office;
Any interested group or individual may multiply this form without asking
authorization from the Plant Variety Protection Office;
