Crisis Protocol: Suicidal/Homicidal Ideation

Asses the clients level of acuity pertaining to suicidiality or homicidality using SLAP method
Document suicidal or homicidal ideation in Epic (electronic medical records and notes)
Consult clients therapist, Saundra (if they have been attending the program)
Alert and consult with Richard Maas, director of the BH unit
With lead approval, admit the client to the assessment center with suicide watch on staff
With lead approval, contact Forsyth County Hospital and check for bed availability
If Forsyth is full, contact Wake Forest Baptist Health ED, Old Vineyard or the State
Once transportation arrives release and discharge the patient to the hospital
Crisis Protocol: Reporting Child/Elder/Dependent Abuse or Neglect

If any type of abuse (mental, physical, sexual or neglect) is disclosed we immediately

document the abuse in Epic.


Following documentation, we contact Richard and inform him of the situation.
After consulting with Richard, we then contact Child Services or Adult Protective Services
and file a formal report.
Documentation is required pertaining to the report, etc.
