Photo copy of current license or certification from PRC. (For Safety Engineer)
Photo copy of certificate(s) of occupational safety and health training/
Seminars sponsored by the Bureau and/or recognized institution.
(Trainings at least 40 Hours)
Processing fee of P 1,000.00 for Temporary Safety Engineer/Inspector;
P 1,500.00 for the Permanent Safety Engineer / Inspector including
Renewals as per Department Administrative Order 2005-08.
