CONTRACT OF LOAN

KNOW ALL MEN BY THESE PRESENTS:

This CONTRACT OF LOAN is made and executed at the City of Cabanatuan, this 29th day of
January 2015, by and between:
I, ABC, Filipino citizen, of legal age, married, and with residence and postal address at
#101 Ukay-ukay Street, Mayapyap, Cabanatuan City, herein referred as LENDER;

AND -

I, 123, likewise legal of age, Filipino citizen, single, with residence and postal address at
948 Patol Street., Mayapyap, Cabanatuan City, hereinafter referred as BORROWER;
WITNESSETH:
That the LENDER and the BORROWER settled that the BORROWER applied for a loan
from the LENDER with the collective amount of SEVEN HUNDRED THOUSAND PESOS
(Php 700,000) and has agreed to extend the loan to the BORROWER upon the terms and
conditions set forth in this Contract as follows:
TERMS AND CONDITIONS
1. That upon signing this contract the BORROWER will start paying the amount of loan every
month equally divided from his debt plus the 10% interest;
2. The amount of loan shall be paid in full until the 29th day of October, 2015;
3. The BORROWER shall pay the LENDER the whole amount of loan of SEVEN HUNDRED
THOUSAND PESOS along with the interest. In case the BORROWER failed to comply with his
obligation to pay in full on the date stated above, he will be liable for necessary obligation and
actions to fulfill this agreement.

This instrument consisting of two pages, including the page on which this
acknowledgement is written, has been signed on each and every page thereof by the concerned
parties and their witnesses, and sealed with my notarial seal.
SUBSCRIBED AND SWORN to before me this 29th day of January, 2015 in the City of
Cabanatuan, Nueva Ecija, Philippines affiant exhibiting before me his Community Tax
Certificate No. 2015728393 issued on January 6, 2015 at Cabanatuan City and will expire on
December 28, 2015.
